About the role
Key responsibilities & impact- Provide direct patient care and comprehensive coordination of care for patients with complex cardiac surgical conditions throughout the continuum of care
- Coordinate surgical procedures and related care with cardiac surgeons, physicians, fellows, advanced practice providers, nurses, ancillary staff, referral sources, and other healthcare partners
- Facilitate patient referrals and appointments
- Coordinate preoperative evaluation and preparation
- Educate patients regarding surgical procedures, expectations, medications, and preoperative and postoperative care
- Coordinate transitions of care and postoperative follow-up
- Perform clinical triage and assess patient needs
- Coordinate prescription refill requests and medication-related patient education
- Identify barriers to care and facilitate timely access to appropriate services
- Collect and document assessment data and develop, evaluate, and update patients' Plans of Care
- Administer medication according to department standards and the Nurse Practice Act
- Verify and evaluate care for consistency with patient- and family-centered care principles, practice standards, and regulatory requirements
- Support evidence-based nursing practices
- Provide patient/family education and discharge planning
- Collaborate and communicate with nursing staff, interdisciplinary teams, executive leaders, and stakeholders
- Document care in the medical record and report variances
- Participate in staff recruitment and retention
- Facilitate development and implementation of evidence-based nursing practice standards
Requirements
What you’ll need- Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing
- Valid RN License
- Minimum of 1 year of Registered Nursing experience
- Current BLS certification or must successfully obtain certification within the first 30 days of employment
- Annual TB Screening, Criminal Background Check, Substance Abuse Testing
- Proof of legal authority to work in the United States on the first day of employment
