About the role
Key responsibilities & impact- Provide direct patient care and comprehensive coordination of care for patients with complex cardiac conditions
- Coordinate care collaboratively with cardiologists, physicians, fellows, advanced practice providers, nurses, ancillary staff, referral sources, and other healthcare partners
- Promote wellness and improve patient outcomes for patients with heart failure and complex cardiovascular conditions
- Educate, support, and assist patients and families with prevention, management, and maintenance of heart failure and related chronic conditions
- Develop and implement individualized plans of care
- Support effective self-management, symptom recognition, disease stability, quality of life, and prevention of avoidable hospitalizations and readmissions
- Collect and document assessment data
- Develop and evaluate patients' Plans of Care
- Administer medication according to standards and the Nurse Practice Act
- Verify and evaluate care for patient- and family-centered care, practice standards, and regulatory compliance
- Support integration of current evidence into nursing practices
- Provide patient/family education and discharge planning
- Communicate care through medical-record documentation
- Report variances in care, systems, and processes
- Participate in staff recruitment and retention
- Facilitate evidence-based nursing practice standards compliant with ANA standards
- Perform other duties as assigned
Requirements
What you’ll need- Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing
- Valid RN License
- Minimum of 1 year of Registered Nursing experience
- Persons must have proof of legal authority to work in the United States on the first day of employment
- Annual TB Screening, Criminal Background Check, Substance Abuse Testing
- Required documents: License or Certificate, list of three professional references, and resume
