About the role

Key responsibilities & impact
  • Provide direct patient care and comprehensive care coordination for patients with complex cardiac surgical conditions across the continuum of care
  • Collaborate with cardiac surgeons, physicians, APPs, and multidisciplinary staff
  • Provide patient education, guidance, and individualized care planning
  • Perform clinical triage
  • Manage referrals and appointment scheduling
  • Coordinate preoperative evaluations
  • Facilitate care transitions and postoperative follow-up
  • Manage prescription refill requests
  • Provide medication education
  • Identify and eliminate barriers to care
  • Collect and document assessment data
  • Develop and evaluate patient Plans of Care
  • Administer medication according to department standards and the Nurse Practice Act
  • Verify and evaluate care for compliance with patient- and family-centered care principles, practice standards, and regulatory requirements
  • Support integration of current evidence into nursing practices
  • Provide patient and family education and discharge planning
  • Communicate and document care in the medical record
  • Report variances in care, systems, and processes
  • Participate in staff recruitment and retention
  • Facilitate development and implementation of evidence-based nursing practice standards

Requirements

What you’ll need
  • Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing
  • Valid RN License
  • Minimum of 1 year of Registered Nursing experience
  • Persons must have proof of legal authority to work in the United States on the first day of employment
  • Annual TB Screening, Criminal Background Check, Substance Abuse Testing
  • Preferred: Two or more years of experience in a clinical environment
  • Preferred: Two or more years of experience in a cardiology or cardiac surgery clinic, cardiovascular ICU, cardiac surgery step-down, or cardiovascular operating room
  • Preferred: Prior experience in a telephone triage role within a surgical or specialty clinic
  • Preferred: Prior experience as a surgical navigator, coordinator, or in a clinical program role
  • Preferred: Knowledge or experience with cardiac surgical patients
  • Preferred: ACLS certification
  • Preferred: Specialty certification such as CCRN, CNOR, CSC, or Cardiac-Vascular Nursing (CV-BC)
