About the role

Key responsibilities & impact
  • Promote wellness, improve outcomes, and support self-care for patients with chronic conditions
  • Provide patient support, education, and assistance with disease prevention and maintenance
  • Develop and implement individual health plans with patients, families, and care providers
  • Coordinate care and follow-up for patients receiving outpatient care
  • Coordinate prescription refill requests and patient education with physician staff
  • Serve as primary coordinator for patients receiving care through the HHT and Vascular Anomalies Program
  • Coordinate diagnostic testing, specialty consultations, treatment plans, and communication among patients, providers, and multidisciplinary teams
  • Provide patient and family education and triage clinical concerns
  • Support complex care coordination across the continuum
  • Support research activities including patient screening, enrollment, data collection, and regulatory requirements
  • Collect and document assessment data and develop patient Plans of Care
  • Evaluate patient progress and administer medication according to standards and the Nurse Practice Act
  • Collaborate with nursing staff, interdisciplinary teams, executive leaders, and other stakeholders
  • Document care in the medical record and report variances
  • Support evidence-based nursing practices, staff recruitment and retention, and nursing practice standards
  • Support program growth through patient and family engagement, educational initiatives, outreach, Family Day, and innovative programs

Requirements

What you’ll need
  • Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing
  • Valid RN License
  • Minimum of one (1) year of Registered Nursing experience
  • Current BLS certification or must successfully obtain certification within the first 30 days of employment
  • Proof of legal authority to work in the United States on the first day of employment
  • Required pre-employment screening: Annual TB Screening, Criminal Background Check, Substance Abuse Testing
  • Resume, License or Certificate, and list of three professional references
