About the role

Key responsibilities & impact
  • Provide direct patient care as a registered nurse
  • Coordinate care and triage patients
  • Collaborate with clinic providers, fellows, staff, nursing teams, interdisciplinary teams, executive leaders, and other stakeholders
  • Work with patient referral sources
  • Facilitate appointments and coordinate transitions of care
  • Provide patient education on complex conditions and discharge planning
  • Develop and implement individual health plans with patients, families, and care providers
  • Collect and document patient assessment data
  • Develop and evaluate patients' plans of care
  • Administer medication according to department standards and the Nurse Practice Act
  • Verify and evaluate care for consistency with patient- and family-centered care principles, practice standards, and regulatory requirements
  • Support integration of current evidence into nursing practices
  • Promote a positive educational environment for students and staff
  • Document care in the medical record
  • Report variances in care, systems, and processes
  • Participate in staff recruitment and retention
  • Facilitate evidence-based nursing practice standards compliant with ANA standards
  • Perform other duties as assigned

Requirements

What you’ll need
  • Graduate of an accredited Nursing Program with Associates Degree, Nursing Diploma, Bachelors in Nursing, or Master's in Nursing
  • Valid RN License
  • Minimum of 1 year Registered Nursing experience
  • Current BLS preferred
  • ACLS certification preferred, or obtain this certification within one year of starting in this role
  • Knowledge or experience in PCMH desired
  • Proof of legal authority to work in the United States on the first day of employment
  • Annual TB screening, criminal background check, and substance abuse testing
