About the role
Key responsibilities & impact- Provide direct patient care as a Registered Nurse
- Coordinate care and triage patients
- Collaborate with clinic providers, fellows, staff, nursing teams, interdisciplinary teams, executive leaders, and other stakeholders
- Work with patient referral sources
- Facilitate appointments and coordinate transitions of care
- Provide patient education on complex conditions and discharge planning
- Develop and implement individual health plans with patients, families, and care providers
- Collect and document patient assessment data
- Develop and evaluate patients' Plans of Care
- Administer medication according to department standards and the Nurse Practice Act
- Verify and evaluate care for consistency with patient- and family-centered care principles, practice standards, and regulatory requirements
- Support integration of current evidence into nursing practice
- Promote a positive educational environment for students and staff
- Document care in the medical record
- Report care, systems, and process variances
- Participate in staff recruitment and retention
- Facilitate development and implementation of evidence-based nursing practice standards compliant with ANA standards
- Perform other duties as assigned
Requirements
What you’ll need- Graduate of an accredited Nursing Program with an Associate's Degree, Nursing Diploma, Bachelor's in Nursing, or Master's in Nursing
- Valid RN License
- Minimum of 1 year of Registered Nursing experience
- Current BLS preferred
- ACLS certification preferred, or obtain this certification within one year of starting in this role
- Knowledge or experience in PCMH desired
- Proof of legal authority to work in the United States on the first day of employment
- Required pre-employment screening: Annual TB Screening, Criminal Background Check, Sex Offender Registry, Substance Abuse Testing
- Required documents: License or Certificate, three professional references, and resume
