About the role

Key responsibilities & impact
  • Deliver comprehensive pharmaceutical care to patients
  • Review medication orders for accuracy, appropriateness, and efficacy
  • Clarify medication orders with prescribers, update orders, and notify involved personnel
  • Document pharmacy interventions in the electronic medical record and clinical decision support system
  • Monitor drug therapy, including dosage, regimen, route, therapeutic duplication, and drug interactions
  • Provide pharmacokinetic consultations and dosing recommendations
  • Prevent, detect, monitor, document, and report adverse drug reactions and medication errors
  • Participate in medication management during medical emergencies
  • Perform admission history and discharge medication review and counseling
  • Provide operational support to Clinical Pharmacists and Clinical Pharmacy Specialists
  • Respond to drug information requests and optimize pharmacotherapy using evidence-based literature
  • Promote rational, cost-effective drug therapy
  • Participate in formulary management and medication procurement
  • Facilitate clinical and operational initiatives
  • Recommend and evaluate drug inventory par levels
  • Document clinical consultations and cost avoidance
  • Develop formulary monographs or medication use evaluations as assigned
  • Mentor pharmacy technicians, students, and residents
  • Supervise certified pharmacy technicians
  • Precept pharmacy students and PGY1/PGY2 residents
  • Participate in research, publications, poster presentations, and other scholarly activities

Requirements

What you’ll need
  • Graduated from an ACPE accredited College of Pharmacy (required)
  • Licensed or license eligible as a pharmacist in the Commonwealth of Kentucky (required)
  • Current board certification by Board of Pharmacy Specialties® (preferred)
  • Current certification in Basic Life Support or must obtain within 90 days from date of hire (required)
  • Current certification in Advanced Cardiac Life Support (ACLS) as determined by pharmacy leadership (required)
  • Complete sterile compounding training, including hazardous medications, and demonstrate competency at the required frequency
  • Complete extemporaneous compounding training, including hazardous medications, and demonstrate competency at the required frequency
  • Ability to communicate and collaborate effectively with patients, physicians, nurses, pharmacists, and allied health professionals
  • Demonstrated competence providing pharmaceutical care for neonatal, pediatric, adolescent, adult, and geriatric patients
  • Comply with HIPAA privacy and security requirements
