About the role

Key responsibilities & impact
  • Approve medication ordering for patients treated at the medical center
  • Oversee and directly contribute to medication compounding, preparation, and dispensing in accordance with state and federal pharmacy laws and regulations
  • Supervise pharmacy technicians and direct their work to ensure appropriate workflow
  • Follow dispensing policies and procedures; dispense medications accurately and promptly
  • Independently resolve problem medication orders
  • Process and validate medication orders using current information-system technology; create and maintain accurate patient profiles
  • Check products requiring pharmacist approval before dispensing
  • Participate in quality assurance, medication-incident reporting and follow-up, MAR reconciliation, and medication adverse-event/error reporting
  • Develop or participate in clinical practice with physicians, nurses, healthcare professionals, and patients as part of an interdisciplinary team
  • Participate in focused clinical initiatives and Medication Use Evaluations
  • Monitor assigned patient populations for clinical progress and recommend therapy adjustments
  • Document clinical activities and interventions
  • Educate Pharmacy staff and healthcare professionals; provide patient teaching when needed
  • Resolve patient-care and operational problems through clear communication and reasonable attempts at resolution
  • Perform charge pharmacist duties, including coordinating shift resources, problem solving, staff conflict resolution, and shift-to-shift communication
  • Contact supervisors when issues exceed expertise or authority
  • Perform other duties as assigned

Requirements

What you’ll need
  • Bachelor's degree in Pharmacy required
  • 1 year of relevant acute care pharmacy experience or successful completion of an accredited pharmacy practice residency required
  • Pharm D preferred
  • Equivalent combination of education and experience accepted
  • Licensed Pharmacist upon hire required
  • Ability to follow pharmacy, departmental, and hospital policies and procedures
  • Ability to accurately and timely dispense medications
  • Ability to resolve problem medication orders
  • Ability to supervise pharmacy technicians
  • Ability to participate in clinical practice, medication-use evaluations, quality assurance, and interdisciplinary patient care
  • Ability to monitor renal/hepatic function, culture results, and other applicable laboratory tests
  • Ability to educate pharmacy staff, healthcare professionals, and patients
  • Ability to perform charge pharmacist duties when scheduled
