About the role

Key responsibilities & impact
  • Provide coordinated care and HIV prevention services
  • Facilitate access to clinical and community resources
  • Help patients navigate complex healthcare needs
  • Coordinate care between obstetric and infectious disease providers for pregnant women living with HIV
  • Collaborate with physicians, community organizations, and multidisciplinary care teams
  • Contribute to grant-funded program initiatives, quality improvement efforts, and patient engagement activities
  • Review and prepare patient charts for clinics
  • Triage patient phone calls, refill medications, and obtain medication/test authorizations
  • Maintain patient laboratory data and client information databases
  • Perform clinical nursing functions, assist physicians/ARNPs with examinations, and administer medications
  • Serve as discharge planner/organizer and clinic coordinator
  • Conduct weekly clinical team meetings and supervise daily clinical functions
  • Establish relationships with referral sources and coordinate outside-agency services
  • Develop and implement individualized client care coordination plans
  • Maintain documentation for mandatory reports and grant funding sources
  • Use office software and electronic medical records
  • Participate in professional development, community engagement, research studies, and special assignments
  • Maintain confidentiality and information security
  • Perform administrative and general office duties

Requirements

What you’ll need
  • Current, unrestricted Registered Nurse (RN) license in the State of Florida
  • Experience in HIV care, HIV prevention, and medical case management
  • Experience providing care to pediatric, adolescent, and young adult populations
  • Phlebotomy and injection administration experience
  • Experience with quality improvement initiatives, data tracking, and/or grant-funded clinical programs
  • Strong written and verbal communication skills, including patient education and public speaking
  • Proficiency with Microsoft 365 applications and Epic electronic medical record (EMR) systems
  • Ability to build collaborative relationships with community partners and multidisciplinary healthcare teams
  • Reliable transportation and ability to travel between clinic sites
  • Ability to manage patient needs remotely, including telephone triage, care coordination, education, and follow-up support
