About the role

Key responsibilities & impact
  • Perform outbound patient calls regarding clinical programs, medication adherence, and closing gaps in care for value-based contracted patients
  • Partner with quality, population health, and pharmacy teams to deliver wrap-around care
  • Call patients to ensure timely refills of chronic disease medications for diabetes, blood pressure, and cholesterol
  • Serve as liaison between patients, clinics, and pharmacies to minimize missed time between refills
  • Interpret patient medical history and communicate providers’ plans to patients
  • Navigate different Electronic Medical Record platforms to review patient information
  • Document patient care activities accurately and comprehensively in electronic records
  • Perform medication reconciliation by comparing patient medication use with medical records and pharmacy fills
  • Request medication refills according to clinic policy and protocol
  • Assist patients with medication access difficulties caused by cost or other barriers
  • Identify patients needing referrals to clinical pharmacists, pharmacy medication programs, care management, clinicians, or Village at Home
  • Follow up persistently with patients, providers, and pharmacies to ensure medications are picked up and taken as directed
  • Coordinate patient care before and after primary care visits
  • Schedule appointments and testing and order screenings according to standardized protocols
  • Maintain current knowledge of quality measure requirements
  • Assist other quality departments as needed per quarter
  • Demonstrate kindness and compassion in patient interactions
  • Participate as a member of the healthcare team to promote positive health outcomes
  • Coordinate with pharmacy staff, primary care office staff, and providers to ensure referrals, orders, and tests are in place
  • Contribute to increasing clinic quality ratings
  • Identify opportunities for continuous process improvement

Requirements

What you’ll need
  • High school diploma or GED equivalent required
  • Certified Medical Assistant, Certified Pharmacy Technician, or equivalent experience in a healthcare setting required
  • Intermediate to advanced level keyboarding skills, including at least 35 WPM, touch typing, and formatting documents required
  • Secure workspace at home, including desk and chair, with minimum background noise required
  • Candidate-supplied home internet access required
  • Minimum internet download speed of 100 Mbps and upload speed of 10 Mbps required
  • Internet must be hard-wired with an ethernet cord; Wi-Fi connection cannot be used
  • 1+ years of experience in a healthcare setting preferred
  • Prior experience using an Electronic Health Record (EHR) preferred
  • Previous call center or telephonic experience preferred
  • Experience with Medicare Advantage plans, insurance, and medication adherence preferred
  • Bilingual English/Spanish preferred
