About the role
Key responsibilities & impact- Provide direct service and daily routine and episodic nursing aide care in a clinically intensive, trauma-informed program
- Assist with activities of daily living, including community membership, vital signs, laundry, toileting, showering, dressing, shaving, wound care, foot care, skin checks, and passive range of motion exercises
- Monitor individuals for changes in physical, mental, or emotional status
- Assist with self-administration of medication and provide for nutritional needs
- Transport and accompany individuals to medical, dental, and other appointments
- Maintain safe, clean, welcoming, respectful, and regulation-compliant environments
- Document nursing care, daily shift/TCM notes, medical records, incident reports, and medication errors
- Communicate client status and care information with managers, colleagues, families, guardians, agencies, and providers
- Safely use and maintain wheelchairs, Hoyer lifts, walkers, glucometers, nebulizers, oxygen, and other medical equipment
- Support participation in home, program, and community events; encourage individual preferences and choices
- Advocate for individuals served and build relationships with individuals, families, guardians, and significant others
- Collaborate with Vinfen departments, outside agencies, and community providers
- Attend staff meetings, trainings, and community meetings
- Act as a positive role model and assist with new staff orientation
- Adhere to Vinfen policies and procedures and perform related duties
Requirements
What you’ll need- Prior experience as a CNA, preferably with people with complex mental health needs
- A high school diploma or equivalent required
- Valid driver's license required; must be at least 21 years of age, have maintained a valid US driver's license for at least six months, and pass a driver's screening background check
- Adequate insurance and safe driving record required when using a personal vehicle
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, and assist clients with activities of daily living
- Ability to operate a computer and office equipment
- Ability to communicate with clients, staff, and external representatives
- CPR within 2 weeks of hire
- First Aid within 2 weeks of hire
- MAP within 150 days of hire
- CPI within 60 days of hire
- CNA certification in Connecticut required
- Ability to work effectively and collaboratively with colleagues
- Ability to accurately communicate verbally and document nursing care
- Knowledge of human, legal, civil rights, community, and other resources
- Knowledge of empowerment and self-advocacy techniques, teaching modalities, and personal computer applications
