About the role

Key responsibilities & impact
  • Provide direct service and daily routine and episodic nursing aide care in a clinically intensive, trauma-informed program
  • Support self-administration of medications according to DMHAS and Vinfen policies and procedures
  • Assist individuals with activities of daily living, including vital signs, laundry, toileting, showering, dressing, shaving, wound care, foot care, skin checks, and passive range-of-motion exercises
  • Monitor individuals for changes in physical, mental, or emotional status
  • Provide for nutritional needs
  • Transport, assist, and accompany individuals to medical, dental, and other appointments
  • Maintain safe, clean, welcoming, respectful, and compliant living and common areas
  • Document nursing care, services, daily shift notes, and TCM notes; maintain accurate medical records and confidential files
  • Complete incident reporting and medication-error documentation
  • Communicate changes in client status to managers, administrators, colleagues, families, guardians, and other staff
  • Safely use and maintain wheelchairs, walkers, Hoyer lifts, glucometers, nebulizers, oxygen, and other medical equipment and supplies
  • Provide opportunities for participation in home, program, and community events
  • Encourage and respect individuals' preferences and choices; advocate for individuals served
  • Develop relationships with individuals served, families, guardians, and other significant contacts
  • Liaise and collaborate with Vinfen departments, outside agencies, and community providers
  • Attend and participate in staff meetings, trainings, and community meetings
  • Act as a positive role model
  • Assist with orientation of new staff
  • Adhere to Vinfen policies and procedures and perform related duties

Requirements

What you’ll need
  • Prior experience as a CNA, preferably with people with complex mental health needs
  • High school diploma or equivalent required
  • CNA certification in the state of Connecticut required
  • CPR within 2 weeks of hire
  • First Aid within 2 weeks of hire
  • MAP within 150 days of hire
  • CPI within 60 days of hire
  • Valid driver's license required
  • If using a personal vehicle, adequate insurance and a safe driving record required
  • At least 21 years of age
  • Valid US driver's license held for at least six months
  • Ability to pass a driver's screening background check
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces or hallways
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to assist clients with activities of daily living
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • Required close, distance, color, peripheral, and depth vision, plus ability to adjust focus
  • Skills and competencies of a CNA
  • Ability to work effectively and collaboratively with colleagues
  • Ability to accurately communicate verbally and document nursing care
  • Sensitivity to cultural, religious, ethnic, disability, and gender issues
  • Knowledge of human, legal, civil rights, community, and other resources
  • Knowledge of empowerment and self-advocacy techniques
  • Knowledge of teaching modalities
  • Ability to balance competing priorities and make independent judgments and decisions
  • Ability to work in a professional and confidential capacity
  • Knowledge of personal computer applications and equipment
