About the role
Key responsibilities & impact- Provide direct service and daily routine and episodic nursing aide care in a clinically intensive, trauma-informed program
- Assist with activities of daily living according to treatment plans, including vital signs, laundry, toileting, showering, dressing, shaving, wound care, foot care, skin checks, and passive range of motion exercises
- Monitor individuals for changes in physical, mental, or emotional status
- Assist with self-administration of medication according to Vinfen policies and procedures
- Provide for nutritional needs
- Transport, assist, and accompany individuals to medical, dental, and other appointments
- Maintain safe, clean, welcoming, respectful, and regulatory-compliant environments
- Document nursing care, services, daily shift notes, and TCM notes; maintain accurate medical records and confidential files
- Follow incident reporting and medication-error procedures
- Communicate client-status changes with managers, colleagues, Vinfen staff, families, guardians, and other appropriate parties
- Safely use and maintain wheelchairs, Hoyer lifts, walkers, glucometers, nebulizers, oxygen, and other medical equipment and supplies
- Provide opportunities for participation in home, program, and community events
- Encourage and respect individuals’ preferences and choices
- Advocate for individuals served
- Develop and maintain relationships with individuals, families, guardians, and other significant contacts
- Liaise and collaborate with Vinfen departments, outside agencies, and community providers
- Attend and participate in staff meetings, trainings, and community meetings
- Act as a positive role model
- Assist with orientation of new staff
- Adhere to Vinfen policies and procedures and perform other assigned duties
Requirements
What you’ll need- Prior experience as a CNA, preferably with people with complex mental health needs
- High school diploma or equivalent required
- CNA certification in the state of Connecticut required
- Valid driver's license required
- At least 21 years of age
- Valid US driver's license maintained for at least six months
- Adequate insurance and safe driving record if using a personal vehicle
- Ability to pass a driver's screening background check
- CPR within 2 weeks of hire
- First Aid within 2 weeks of hire
- MAP within 150 days of hire
- CPI within 60 days of hire
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways
- Ability to assist clients with activities of daily living
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Close, distance, color, peripheral, and depth vision, plus ability to adjust focus
- Skills and competencies of a CNA
- Ability to communicate verbally and document nursing care accurately and effectively
- Sensitivity to cultural, religious, ethnic, disability, and gender issues
- Knowledge of human, legal, civil rights, community, and other resources
- Knowledge of empowerment and self-advocacy techniques and teaching modalities
- Ability to balance competing priorities and make independent judgments and decisions
- Ability to work professionally, confidentially, effectively, and collaboratively
- Knowledge of personal computer applications and equipment
