About the role
Key responsibilities & impact- Participate as a member of the intervention team for assigned Persons Served
- Deliver Treatment Plan interventions addressing daily living, medication administration, symptom reduction, health and wellbeing, employment, education, socialization, and recreation
- Help Persons Served identify and access community resources, social supports, natural supports, and mainstream opportunities
- Provide staff training and guidance on community integration practices
- Help Persons Served obtain benefits and entitlements including financial assistance, childcare, education, employment, energy assistance, food programs, healthcare, legal assistance, and transportation
- Train staff on benefits and entitlements and maintain the Community Resource Book
- Support Persons Served in preparing for, seeking, and maintaining employment or education
- Support symptom self-management through Safety Plans and medication education
- Provide trauma-informed, recovery-oriented, person-centered, culturally competent, and strengths-based interventions
- Provide evidence-based interventions including Motivational Interviewing, Harm Reduction, SBIRT, Housing First, Stages of Change, WRAP, Seeking Safety, and Cognitive Behavioral interventions
- Promote engagement, housing permanency, community tenure, self-sufficiency, and successful service completion
- Document Persons Served needs and services
- Promote access to healthcare services
- Participate in the Community Connector Community of Practice Workgroup
- Perform other related duties as assigned
Requirements
What you’ll need- Unlicensed Master’s level or very experienced direct care staff with Bachelor’s degree preferred
- Must have access to an operational and insured vehicle and be willing to use it to transport PS
- Valid driver’s license required at employment
- At least 21 years of age
- Valid US driver’s license maintained for at least six months
- Pass a driver’s screening background check
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer and office equipment such as a calculator, copier, and printer
- Ability to assist PS with tasks of daily living
- Ability to communicate with PS, staff, and external representatives
- CPR required within first two weeks of hire
- First Aid required within two weeks of hire
- MAP required within 150 days of hire
- Knowledge of community inclusion principles, benefits and entitlement resources, assessment skills, documentation standards, and personal computer applications
- Ability to use Motivational Interviewing, Harm Reduction, SBIRT, Housing First, Stages of Change, and Cognitive Behavioral interventions
- Ability to collaborate effectively with team members and work professionally and confidentially
