About the role
Key responsibilities & impact- Coordinate completion of intake, assessment, and treatment plans
- Conduct ongoing risk assessments and develop crisis management, relapse prevention, and harm reduction strategies with the team clinician
- Drive referrals to community and social services aligned with members' needs and goals
- Collaborate with PCPs, providers, community resources, emergency services, mobile crisis intervention teams, outpatient providers, ICT, and hospital staff
- Assist members with daily living skills, benefits, housing, health care, transportation, and essential health care appointments
- Manage care transitions and coordinate safe discharge plans
- Complete service notes and case documentation in the electronic health record for accurate and timely billing
- Participate in multidisciplinary team meetings and assigned training
- Provide pre-tenancy supports, including housing preferences, budgeting, subsidized housing applications, and temporary housing resources
- Provide tenancy transition services, including lease negotiations, accommodations, budgeting, rental payment plans, furniture, equipment, and in-home health services
- Provide tenancy-sustaining supports, including landlord negotiations, benefit programs, lease-compliance education, and legal and financial resources
- Conduct outreach, housing assessments, targeted housing plans, and in-home remediation assessments
- Coordinate home modifications to support members' health, safety, welfare, and independent community living
- Use evidence-based and promising practices, including the Housing First approach
- Develop relationships with agencies serving people experiencing homelessness or at-risk tenancies
- Use recovery-oriented, person-centered, culturally competent, strengths-based, trauma-informed, and culturally responsive approaches
- Perform other related duties as assigned
Requirements
What you’ll need- Experience with and knowledge of affordable housing services and resources
- Experience working with people living with Serious Mental Illness (SMI) and/or Substance Use Disorder (SUD)
- Must have access to an operational and insured vehicle and be willing to use it to transport members
- A Bachelor's or Master's degree in Mental Health, Social Services, or Criminal Justice preferred
- In some cases, experience may be considered a substitute for academic training
- Valid driver's license required at employment
- Must be at least 21 years of age
- Must have maintained a valid US driver's license for at least one year
- Must pass a driver's screening background check
- Must possess and maintain adequate personal vehicle insurance if using a personal vehicle
- Must maintain a safe driving record subject to annual checks
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces or hallways
- Must be able to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to assist clients with activities of daily living
- Ability to operate a computer and office equipment
- CPR within 2 weeks of hire
- First Aid within 2 weeks of hire
- Preference given to bi-lingual/bi-cultural applicants and those with lived experience of behavioral health conditions
