About the role

Key responsibilities & impact
  • Promote the full integration and functioning of Persons Served to live successfully in the community
  • Provide frontline, community-based care, crisis response, and rehabilitation support
  • Serve as a bridge connecting Persons Served with community partners, emergency services, and first responders
  • Maintain a trauma-informed environment promoting safety and recovery
  • Answer telephones, direct calls, assist with phone messages, and maintain phone listings
  • De-escalate active conflicts, manage emergency situations, and provide immediate on-the-ground support
  • Connect clients and community members to housing, mental health counseling, substance abuse treatment, and victim services
  • Promote trauma-informed, person-served, strengths-based, and person-first service delivery
  • Design and implement community-based, trauma-informed violence and disruptive-behavior prevention strategies with clinical leadership
  • Facilitate warm hand-offs to Living Room and/or Sober Support areas
  • Track client flow to increase accessibility and problem-solve engagement and connection barriers
  • Coordinate care with program staff, supervisors, clinicians, and emergency personnel
  • Support incident response by gathering information, notifying appropriate parties, and completing documentation
  • Liaise between the public, local organizations, and legal or law enforcement agencies
  • Maintain documentation and records
  • Participate in supervision and staff meetings
  • Perform other related duties as assigned

Requirements

What you’ll need
  • 1-2 years’ experience in human services, preferably in a behavioral health community-based outpatient or acute/diversionary behavioral health setting
  • Personal lived experience of a mental health or substance use condition strongly preferred
  • Linguistic, social, cultural, ethnic, and racial demographics reflective of the community served strongly preferred
  • High school diploma or equivalent required
  • Bachelor’s degree in social work, psychology, criminal justice, or another related field preferred
  • In some cases, experience may be substituted for academic training
  • Excellent customer service, interpersonal, written, and verbal communication skills
  • Ability to develop effective documentation and maintain satisfactory working relationships
  • Demonstrated competence serving culturally, ethnically, and socially diverse populations
  • Understanding of social determinants of health and commitment to crisis intervention in whole-person care
  • Understanding of trauma, co-occurring disorders, mental health disorders, and substance use disorders strongly preferred
  • Valid US driver's license presented at employment
  • At least 21 years of age and valid US driver's license maintained for at least six months
  • Ability to pass a driver's screening background check and maintain a safe driving record if driving
  • Adequate insurance if using a personal vehicle
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, and assist clients with daily living tasks
  • Ability to operate a computer, calculator, copier, and printer
  • CPR within 2 weeks of hire
  • First Aid within 2 weeks of hire
  • CPAA required within 6 months of hire
