About the role

Key responsibilities & impact
  • Provide instruction and skill building in self-help, sensorimotor, communication, social development, independent living, behavior development, affective development, and wellness development
  • Assist with case management responsibilities
  • Provide input for interdisciplinary team reviews
  • Assist in developing Day Habilitation Service Plans (DHSPs)
  • Maintain contact with family members, funding sources, staff, and interdisciplinary team members
  • Implement clinical and daily plans and recommend or implement program improvements
  • Maintain daily documentation, data collection, and progress notes
  • Work individually and in groups with persons served to develop skills
  • Ensure members remain engaged in meaningful activities and offer choices
  • Communicate with management regarding treatment and programs
  • Participate in case management and other teams
  • Support members in crisis management using Positive Behavior Support (PBS) and Safety Care
  • Assist with staff training as needed
  • Maintain program-area upkeep and monitor, clean, and store supplies and equipment
  • Advocate for persons served and support community integration through community and volunteer opportunities, reverse integration, and friendships
  • Perform other related duties as required

Requirements

What you’ll need
  • One year of relevant work experience/training
  • A high school diploma or equivalent is required
  • Valid driver's license presented at employment
  • At least 21 years of age
  • Valid US driver's license maintained for at least six months
  • Ability to pass a driver's screening background check
  • Adequate insurance and safe driving record if using a personal vehicle
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and perform physically active duties
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to assist clients with daily living tasks
  • Ability to communicate with clients, staff, and external representatives
  • Ability to perform routine living activities including cleaning, meal preparation, vacuuming, shoveling, and grocery shopping
  • CPR within 2 weeks of hire
  • First Aid within 2 weeks of hire
  • New Employee Training (NET)
  • Safety Care within 90 days of hire
  • Knowledge of advocacy techniques, applied behavioral analysis, communication styles, empowerment and self-advocacy, assessment practices, participatory planning, personal management, independent living management, and crisis intervention techniques
  • Ability to work independently and as part of an interdisciplinary team
  • Presentation, persuasive, and collaborative skills
  • Ability to provide a positive role model
