About the role
Key responsibilities & impact- Provide case management, treatment planning, direct support, advocacy, family contact, and related documentation
- Supervise, train, coach, mentor, and support Program Instructors, interns, volunteers, relief staff, and other staff
- Assist with implementation of Day Habilitation Support Plans, recommended schedules, and plans
- Participate in development of DHSPs, Safety Assessments, and other assessments
- Manage a caseload of individuals served
- Implement, monitor, and evaluate clinical, adaptive skills, communication, and other programs
- Recommend or implement changes to support continuous improvement of skills
- Advocate for individuals served and promote self-expression, self-reliance, creativity, and human rights
- Attend consultation meetings with clinical and paraprofessional staff
- Work directly with individuals individually and in groups, including personal care and activities of daily living
- Assess functional skills and identify therapeutic interventions with professional and paraprofessional staff
- Maintain approved contact with family members and residential staff
- Develop and participate in age-appropriate activities and groups
- Document data, transfer data to daily charts, and complete monthly progress reviews
- Maintain program area, supplies, and equipment; ensure equipment is clean, safe, and in good repair
- Complete training in applied behavioral analysis, teaching techniques, and Positive Behavior Support
- Complete six-month progress summaries and annual assessments
- Organize individual and group activities and ensure meaningful skill-building engagement
- Communicate with management and participate actively in the DHSP team
- Notify management of emergencies
- Function as Human Rights Officer and Fire Safety Officer if assigned
- Perform other related duties as required
Requirements
What you’ll need- High School diploma or GED required
- A bachelor's degree in Social Work, Psychology, or related field preferred, but experience may substitute for education
- 1-2 years of related experience
- Valid US driver's license presented at employment
- At least 21 years of age
- Valid US driver's license maintained for at least six months
- Pass a driver's screening background check
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways
- Ability to assist persons served with activities of daily living
- Ability to remain in a stationary position 50% of the time
- Ability to speak, hear, and communicate with persons served, staff, and external representatives
- Specific vision abilities including close, distance, color, peripheral, and depth perception, and ability to adjust focus
- Ability to assist with cleaning, meal preparation, vacuuming, shoveling, and grocery shopping
- CPR required within two weeks of hire
- First Aid required within two weeks of hire
- Safety Care required within 90 days of hire
- NET required
- Adequate insurance and safe driving record if using a personal vehicle
