About the role

Key responsibilities & impact
  • Provide direct residential services in a GLE/SIE setting to help Persons Served overcome barriers and prepare for independent living
  • Implement treatment plan support interventions under the guidance of the Team LPHA Clinician
  • Assist the Program Director with orienting new residents and performing day-to-day program assignments
  • Collaborate with Integrated Team members to address Persons Served’s needs
  • Liaise with families and support medical and psychiatric care
  • Administer, document, store, and dispose of medications according to MAP policies and Vinfen protocols
  • Provide crisis intervention and access on-call backup when needed
  • Assume Medication Officer, Safety Officer, and Human Rights Officer roles as assigned
  • Schedule and facilitate weekly resident group meetings, including menu input
  • Perform grocery shopping, meal preparation, physical site maintenance, and infection control inventory
  • Encourage resident social interaction and maintain a safe, comfortable, clean, and collaborative environment
  • Advocate for Persons Served and serve as a resource for neighbors
  • Complete required documentation, incident reports, surveillance testing organization, and quality improvement activities
  • Participate in trainings, supervision, and staff meetings
  • Support evidence-based practices including Motivational Interviewing, Harm Reduction, Housing First, Stages of Change, WRAP, Seeking Safety, and Cognitive Behavioral interventions

Requirements

What you’ll need
  • High school diploma or equivalent required
  • Associate’s degree in a human service discipline preferred
  • One year as a residential counselor working with Persons Served facing mental health challenges preferred
  • One to two years of job-specific experience in the human services field preferred
  • Valid driver's license required at employment
  • Adequate personal vehicle insurance if using a personal vehicle
  • Safe driving record and successful driver's screening background check
  • At least 21 years of age
  • Valid US driver's license maintained for at least six months
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and maneuver through narrow spaces
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to assist Persons Served with activities of daily living
  • Ability to speak, hear, and communicate with Persons Served, staff, and external representatives
  • CPR certification required within two weeks of hire
  • First Aid certification required within two weeks of hire
  • MAP certification required within 150 days of hire
  • Knowledge of personal computer applications and equipment
  • Ability to work professionally and confidentially
