About the role

Key responsibilities & impact
  • Participate in comprehensive physical-health assessments for people with brain injuries, intellectual and developmental disabilities, psychiatric conditions, and complex health needs
  • Develop and implement person-centered treatment care plans
  • Provide nursing care and assistance with activities of daily living, including showering, dressing, mouth care, diabetes care, wound care, GT/JT care, and nutritional support
  • Monitor individuals for changes in status, perform individualized assessments, and provide skilled nursing interventions
  • Administer medications and treatments as ordered by primary care providers under Medication Administration Plan guidelines
  • Provide transportation and accompany individuals to medical, dental, and other appointments
  • Ensure nutritional needs and timely ordering of medical, pharmacological, nutritional supplies, and equipment
  • Apply behavioral strategies and prescribed clinical plans
  • Supervise C.N.A.s and ensure timely, accurate care aligned with ISP goals and quality standards
  • Provide nursing education and training to C.N.A.s
  • Communicate with program leadership, primary care providers, families, guardians, employees, agencies, community providers, and neighbors
  • Use emergency and on-call systems when appropriate
  • Document nursing care, treatments, medication administration, shift notes, communication, treatment plans, medical records, and incident reports
  • Maintain safe, clean, welcoming, compliant environments and safely store medications and treatment supplies
  • Maintain clients’ medical equipment and supplies, including wheelchairs, walkers, Hoyer lifts, and hospital beds
  • Support community outings and events and respect individual preferences and choices
  • Advocate for people served and collaborate with Vinfen departments and outside providers
  • Participate in treatment-plan, ISP, and other planning meetings
  • Build relationships with individuals served, families, guardians, neighbors, and other significant people
  • Act as a positive role model, participate in Vinfen training, and attend educational opportunities
  • Comply with accreditation, licensing, fire, safety, and other regulatory requirements
  • Perform other assigned duties

Requirements

What you’ll need
  • High school diploma or GED required
  • Current L.P.N. license in the state of service required
  • 1–2 years supervised practical experience, including internships, in a clinical or rehabilitation setting preferred/helpful but not required
  • Experience working with people with psychiatric conditions and/or co-occurring disorders strongly preferred
  • Experience working with people with developmental and/or psychiatric disabilities and medical illnesses and other co-occurring disorders preferred
  • Strong time management and writing skills
  • Minimum age of 21
  • Current, valid US driver’s license; license tenure requirements stated as at least 6 months and elsewhere at least one year
  • Adequate insurance and safe driving record required when using a personal vehicle
  • Must obtain CPR and First Aid certification within 2 weeks of employment
  • Must obtain MAP certification within 150 days of employment
  • Must obtain Safety Care certification within 90 days of hire
  • Must pass CORI, reference, multi-state background, driving record, and driver screening checks
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and lift at least 25 pounds using proper techniques or a two-person lift
  • Ability to operate computers and office equipment
  • Ability to assist clients with activities of daily living
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • Knowledge of LPN practice standards and intervention techniques
  • Knowledge of developmental disabilities, evidence-based practices, assessment practices, documentation standards, community health and mental health services, and personal computer applications
  • Ability to supervise and mentor C.N.A.s
  • Ability to make independent judgments and decisions and work professionally and confidentially
  • Preference given to bilingual/bicultural applicants and applicants with lived experience
