About the role
Key responsibilities & impact- Implement service plan interventions with persons served
- Provide direct training and support in daily living skills, symptom management, medication management, money management, cooking, cleaning, recreation, personal hygiene, and social skills
- Maintain and update documentation in individual and program records
- Advocate for persons served
- Communicate with co-workers, supervisors, other service providers, and funding agency personnel
- Escort persons served into the community using public transportation, company vehicles, or personal vehicles
- Monitor persons served, provide crisis intervention and counseling, and report incidents
- Perform assigned housecleaning tasks
- Attend in-service trainings and staff meetings
- Provide program coverage, including overnights, days, evenings, weekends, and holidays
- Follow program and company policies, procedures, standards, and regulations
- Store, administer, document, and dispose of medication according to MAC requirements, funding source regulations, and company policy
- Assume on-call duties in some programs
- Provide outreach to persons served living in their own homes
- Perform other assigned duties
Requirements
What you’ll need- At least one year of work experience in human services with adults with psychiatric illness and/or developmental disabilities
- High school diploma or equivalent required
- Bachelor’s degree in counseling psychology, human services, or related field preferred/optional
- Experience may be substituted for academic training in some cases
- Valid driver's license required at employment
- At least 21 years of age
- Valid US driver's license maintained for at least six months
- Adequate insurance and safe driving record if using a personal vehicle
- Pass a driver's screening background check
- CPR required within two weeks of hire
- First Aid required within two weeks of hire
- MAC follows CT guidelines
- Safety Care required within 60 days of hire
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and assist clients with daily living tasks
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Ability to work independently and as part of an interdisciplinary team
- Knowledge of case management, advocacy, communication, crisis intervention, assessment, planning, household management, and personal management skills
- Sensitivity to cultural, religious, racial, disability, and gender issues
