About the role
Key responsibilities & impact- Contribute to high-quality case management services for persons served
- Implement service plan interventions
- Provide direct training and support for daily living skills, including symptom, medication, and money management, cooking, cleaning, recreation, hygiene, and social skills
- Maintain and update individual and program records
- Advocate for persons served
- Communicate with coworkers, supervisors, service providers, and funding agency personnel
- Escort persons served in the community using public transportation, company vehicles, or personal vehicles
- Monitor persons served and provide crisis intervention and counseling as needed
- Report incidents to the on-call manager or supervisor
- Perform assigned housecleaning tasks
- Attend in-service trainings and staff meetings
- Provide program coverage, including overnights, days, evenings, weekends, and holidays
- Follow program, company, funding-source, and regulatory policies and standards
- Store, administer, document, and dispose of medication consistent with MAC requirements, funding-source regulations, and company policy
- May assume on-call duties
- May provide outreach to persons served living in their own homes
- Perform other assigned duties
Requirements
What you’ll need- At least one year of work experience in human services with adults with psychiatric illness and/or developmental disabilities
- High school diploma or equivalent required
- Bachelor’s degree in counseling psychology, human services, or related field preferred/possibly substituted by experience
- Valid driver's license presented at employment
- At least 21 years of age
- Valid US driver's license maintained for at least six months
- Adequate insurance and safe driving record when using a personal vehicle
- Ability to pass a driver's screening background check
- CPR certification required within two weeks of hire
- First Aid certification required within two weeks of hire
- MAC follows Connecticut guidelines
- Safety Care certification required within 60 days of hire
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and perform physically active duties
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to assist clients with activities of daily living
- Ability to communicate with clients, staff, and external representatives
- Ability to work independently and as part of an interdisciplinary team
- Sensitivity to cultural, religious, racial, disability, and gender issues
- Knowledge of advocacy, communication styles, teaching, human/legal/civil rights and community resources, crisis intervention, assessment, and participatory planning techniques
