About the role

Key responsibilities & impact
  • Implement service plan interventions with persons served
  • Provide direct training and support in daily living skills, including symptom, medication, and money management, cooking, cleaning, recreation, personal hygiene, and social skills
  • Maintain and update individual and program records
  • Advocate for persons served
  • Communicate with co-workers, supervisors, service providers, and funding agency personnel
  • Escort persons served into the community using public transportation, company vehicles, or personal vehicles
  • Monitor status and provide crisis intervention and counseling as needed
  • Report incidents to the on-call manager or supervisor
  • Perform assigned housecleaning tasks
  • Attend in-service trainings and staff meetings
  • Provide program coverage, including overnights, days, evenings, weekends, and holidays
  • Follow program and company policies, procedures, standards, and regulations
  • Store, administer, document, and dispose of medication consistent with MAC requirements, funding regulations, and company policy
  • May assume on-call duties
  • Provide outreach to persons served living in their own homes
  • Perform other assigned duties

Requirements

What you’ll need
  • At least one year of work experience in human services with adults with psychiatric illness and/or developmental disabilities
  • High school diploma or equivalent required
  • Bachelor’s degree in counseling psychology, human services, or related field
  • In some cases, experience may be substituted for academic training
  • Valid driver's license presented at employment
  • At least 21 years of age
  • Valid US driver's license maintained for at least six months
  • Adequate insurance and safe driving record when using a personal vehicle
  • Ability to pass a driver's screening background check
  • CPR required within two weeks of hire
  • First Aid required within two weeks of hire
  • MAC follows CT guidelines
  • Safety Care required within 60 days of hire
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and perform physically active duties
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to assist clients with activities of daily living
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • Required vision abilities include close, distance, color, peripheral, and depth perception, plus ability to adjust focus
  • Ability to work independently and as part of an interdisciplinary team
  • Knowledge of advocacy, communication, teaching, crisis intervention, assessment, planning, household management, and community resources
