About the role
Key responsibilities & impact- Participate as a member of the intervention team for assigned Persons Served
- Perform Treatment Plan interventions, skills teaching, and community-resource support under LPHA Clinician direction
- Deliver skills-building interventions involving daily living, medication administration, symptom reduction, health and wellbeing, employment, education, socialization, and recreation
- Provide support and supervision to Persons Served as needed
- Attend medical, psychiatric, and other appointments with Persons Served
- Develop and maintain professional relationships with community resources and service providers
- Adhere to Medication Administration Program policies and procedures
- Provide crisis intervention when needed
- Use trauma-informed, recovery-oriented, person-centered, culturally competent, and strengths-based approaches
- Support symptom self-management through Safety Plans and medication-administration education
- Participate in the regional ACCS Outreach Worker Community of Practice Workgroup
- Support evidence-based practices including Motivational Interviewing, Harm Reduction, Housing First, Stages of Change, Seeking Safety, WRAP, and Cognitive Behavioral interventions
- Promote engagement, safe housing, community tenure, self-sufficiency, and successful service completion
- Document Persons Served needs and services thoroughly
- Promote access to healthcare, employment, and education services
- Help orient new staff and Persons Served to ACCS services
- Attend assigned trainings and perform other related duties
Requirements
What you’ll need- A high school diploma or equivalent is required
- One to two years related experience working with people with psychiatric disability and/or co-occurring disorders preferred
- Access to an operational and insured vehicle and willingness to use it to transport PS
- Valid driver's license required at time of employment
- At least 21 years of age
- Valid US driver's license maintained for at least six months
- Adequate insurance and safe driving record if using a personal vehicle
- Ability to pass a driver's screening background check
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and perform physically active work
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to assist Persons Served with activities of daily living
- Ability to remain stationary 50% of the time as needed
- Ability to speak, hear, and communicate with Persons Served, staff, and external representatives
- Close, distance, color, peripheral, and depth vision, plus ability to adjust focus
- CPR certification required within first two weeks of hire
- First Aid certification required within two weeks of hire
- MAP certification required within 150 days of hire
- Knowledge of recovery-oriented, person-centered, strengths-based, trauma-informed, and culturally competent services
- Knowledge of documentation standards, functional assessment practices, engagement strategies, and personal computer applications
- Ability to adhere to Motivational Interviewing, Harm Reduction, SBIRT, Housing First, Stages of Change, and Cognitive Behavioral intervention standards
