About the role
Key responsibilities & impact- Participate as a member of the intervention team for assigned Persons Served
- Perform non-clinical Treatment Plan interventions, skills teaching, and community-resource support under the direction of a Team LPHA Clinician
- Deliver skills-building interventions involving daily living, medication administration, symptom reduction, health and wellbeing, employment, education, socialization, and recreation
- Provide support and supervision to Persons Served and attend medical, psychiatric, and other appointments as needed
- Develop and maintain professional relationships with community resources and service providers
- Adhere to Medication Administration Program policies and procedures
- Provide crisis intervention when needed
- Use trauma-informed, recovery-oriented, person-centered, culturally competent, and strengths-based approaches
- Support symptom self-management through Safety Plans and medication administration services and education
- Participate in the regional ACCS Outreach Worker Community of Practice Workgroup for professional development and mentoring
- Support evidence-based practices including Motivational Interviewing, Harm Reduction, Housing First, Stages of Change, Seeking Safety, WRAP, and Cognitive Behavioral interventions
- Promote engagement, safe housing, community tenure, self-sufficiency, and successful service completion
- Document Persons Served needs and services thoroughly
- Promote access to healthcare, employment, and education services
- Help orient new staff and Persons Served to ACCS services
- Attend assigned trainings and perform other related duties
Requirements
What you’ll need- A high school diploma or equivalent is required
- One to two years related experience working with people with psychiatric disability and/or co-occurring disorders preferred
- Must have access to an operational and insured vehicle and be willing to use it to transport PS
- Must possess and maintain adequate insurance when using a personal vehicle
- Must maintain a safe driving record subject to annual checks
- A valid driver's license must be presented at the time of employment
- Must be at least 21 years of age
- Must have maintained a valid US driver's license for at least six months
- Must pass a driver's screening background check
- CPR required within first two weeks of hire
- First Aid required within two weeks of hire
- MAP required within 150 days of hire
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and perform physically active work
- Must be able to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer and office equipment
- Ability to assist Persons Served with activities of daily living
- Ability to speak, hear, and communicate with Persons Served, staff, and external representatives
- Knowledge of community resources, engagement strategies, functional assessment, documentation standards, and recovery-oriented, trauma-informed, culturally competent services
- Ability to provide Treatment Plan interventions and use Motivational Interviewing, Harm Reduction, SBIRT, Housing First, Stages of Change, and Cognitive Behavioral interventions
