About the role

Key responsibilities & impact
  • Participate as a member of the intervention team for assigned Persons Served
  • Perform Treatment Plan skills teaching and help PS access community resources
  • Provide support and supervision as needed
  • Deliver interventions addressing daily living, medication administration, symptom reduction, health and wellbeing, employment, school, socialization, and recreation
  • Attend medical, psychiatric, and other appointments with PS
  • Develop and maintain professional relationships with community resources and service providers
  • Adhere to Medication Administration Program policies and procedures
  • Provide crisis intervention when needed
  • Use trauma-informed, recovery-oriented, person-centered, culturally competent, and strengths-based approaches
  • Support symptom self-management through Safety Plans and medication administration education
  • Participate in the regional ACCS Outreach Worker Community of Practice Workgroup
  • Support evidence-based practices including Motivational Interviewing, Harm Reduction, Housing First, Stages of Change, Seeking Safety, WRAP, and Cognitive Behavioral interventions
  • Promote engagement, safe housing, community tenure, self-sufficiency, and successful service completion
  • Document PS needs and services thoroughly
  • Promote access to healthcare, employment, and education services
  • Help orient new staff and PS to the ACCS service
  • Attend assigned trainings and perform other related duties
  • Transport PS using a Vinfen van or personal vehicle when required

Requirements

What you’ll need
  • A high school diploma or equivalent is required
  • One to two years related experience working with people with psychiatric disability and/or co-occurring disorders preferred
  • Access to an operational and insured vehicle and willingness to use it to transport PS
  • Valid driver's license presented at time of employment
  • At least 21 years of age
  • Valid US driver's license maintained for at least six months
  • Ability to pass a driver's screening background check
  • CPR required within first two weeks of hire
  • First Aid required within two weeks of hire
  • MAP required within 150 days of hire
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and assist PS with daily living tasks
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer and office equipment
  • Ability to speak, hear, and communicate with PS, staff, and external representatives
  • Required vision abilities include close, distance, color, peripheral, and depth perception, plus focus adjustment
  • Knowledge of community resources, PS engagement strategies, functional assessment, documentation standards, recovery-oriented and trauma-informed services, and computer applications
  • Ability to work independently and as part of a team, make independent judgments, balance competing priorities, and work confidentially
