About the role

Key responsibilities & impact
  • Provide direct support on an as-needed basis to adults with mental health conditions or behavioral health challenges in small group residential settings or individual living arrangements
  • Work with people served, families, friends, employers, and support staff to build independence skills and increase quality of life
  • Develop, implement, and document individual treatment support plans
  • Provide rehabilitative and behavioral interventions and monitoring
  • Schedule appointments and monitor progress
  • Provide transportation for outings such as shopping and doctor’s appointments
  • Serve as a role model and support community integration
  • Assess physical and emotional status and report changes to supervisors
  • Ensure program site safety and maintenance
  • Assist with cooking, meal planning, grocery shopping, housekeeping, lawn mowing, snow shoveling, and garbage removal
  • Store, administer, document, and dispose of medication in accordance with MAP/MAC requirements, regulations, and company policy
  • Complete communication logs, community membership logs, chore schedules, timesheets, and application forms
  • Perform crisis intervention as needed
  • Provide current contact information to the Relief Pool Coordinator or Human Resources staff
  • Perform other related duties as required

Requirements

What you’ll need
  • One to three months experience in a human services setting; in some cases, experience may be substituted for academic training
  • A high school diploma or equivalent is required
  • May be called on to drive for Vinfen using either a Vinfen van or personal vehicle
  • Valid US driver’s license presented at employment
  • At least 21 years of age
  • Maintained a valid US driver’s license for at least one year
  • Pass a driver’s screening background check, subject to annual checks
  • Maintain a safe driving record if driving
  • Adequate insurance if using a personal vehicle
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, and assist clients with activities of daily living
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • First Aid required within two weeks of hire
  • CPR required within two weeks of hire
  • MAC required, following CT guidelines
  • Safety Care required within 60 days of hire
  • Knowledge and use of advocacy, communication, empowerment, self-advocacy, physical personal management, household management, and crisis intervention techniques
  • Sensitivity to cultural, religious, racial, disability, and gender issues
  • Teaching skills
  • Ability to document interventions
  • Ability to work independently and as part of an interdisciplinary team
  • Ability to provide role model behavior
