About the role
Key responsibilities & impact- Assist adults with brain injuries and complex health needs with daily routine or episodic nursing aide care
- Provide assistance with activities of daily living according to treatment plans, including community membership, vital signs, laundry, toileting, showering, dressing, shaving, wound care, foot care, skin checks, and passive range of motion exercises
- Perform rehabilitative and behavioral interventions and monitoring
- Schedule appointments and monitor client progress
- Support clients in becoming fully integrated into their communities
- Take and document temperatures, pulse, respirations, blood pressure, skin care, toileting, and passive range of motion exercises as ordered
- Implement and document individual treatment support plans
- Store, administer, document, and dispose of medications according to MAP requirements, regulations, and company policy
- Transport, assist, and accompany clients to medical, dental, and other appointments
- Maintain a safe, clean, welcoming, and respectful environment meeting safety and regulatory standards
- Advocate for clients and report changes in medical condition or mental status to supervisors
- Participate in trainings and staff meetings
- Safely use and maintain wheelchairs, Hoyer lifts, walkers, glucometers, nebulizers, oxygen, and other medical equipment
- Complete service delivery, MAP, and administrative documentation
- Contribute to program quality improvement goals
- Use crisis intervention techniques and complete incident documentation when needed
- Participate in cooking, meal planning, grocery shopping, housekeeping, lawn mowing, snow shoveling, and garbage removal
- Adhere to company policies, safe work practices, universal precautions, exposure controls, accreditation standards, licensing standards, and regulations
- Perform other related duties as required
Requirements
What you’ll need- One to two years of job-specific experience in human services field preferred
- Experience working with individuals with brain injury preferred
- A high school diploma or equivalent is required
- Valid driver’s license must be presented at the time of employment
- Must be at least 21 years of age if driving
- Must have maintained a valid US driver’s license for at least one year if driving
- Must pass a driver’s screening background check if driving
- Must maintain a safe driving record if driving
- Must possess and maintain adequate insurance if using a personal vehicle
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
- Must be able to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer and office equipment such as a calculator, copier, and printer
- Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways
- Ability to assist clients with activities of daily living
- Ability to remain in a stationary position 50% of the time as needed
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Specific vision abilities including close, distance, color, peripheral, and depth vision, and ability to adjust focus
- First Aid certification required within two weeks of hire
- CPR certification required within two weeks of hire
- MAP certification required within 150 days of hire
- Safety Care certification required within 90 days of hire
- CNA in state of service required
