About the role

Key responsibilities & impact

  • Teach skills that enhance independence and quality of life
  • Develop, implement, and document individual treatment support plans
  • Provide rehabilitative and behavioral interventions and monitoring
  • Schedule appointments and monitor progress
  • Provide transportation and other related support
  • Model and support appropriate behaviors
  • Assist persons served in becoming fully integrated into their communities
  • Assess physical and emotional status and report changes to a supervisor
  • Ensure program site safety and maintenance
  • Oversee and participate in daily living and cleaning tasks, including cooking, meal planning, grocery shopping, housekeeping, lawn mowing, snow shoveling, and garbage removal
  • Comply with corporate and divisional policies and safe work practices
  • Store, administer, document, and dispose of medication consistent with MAP requirements, funding regulations, and company policy
  • Complete communication logs, community membership logs, chore schedules, timesheets, and application forms
  • Perform crisis intervention as needed
  • Provide current contact information to the Relief Pool Coordinator or Human Resources staff
  • Perform other related duties as required

Requirements

What you’ll need
  • One to three months experience in a human services setting
  • Experience working with individuals with brain injury preferred
  • A high school diploma or equivalent is required
  • Valid US driver’s license if driving for Vinfen
  • At least 21 years of age if driving
  • Valid US driver’s license maintained for at least six months if driving
  • Ability to pass a driver’s screening background check if driving
  • Adequate insurance if using a personal vehicle
  • First Aid required within two weeks of hire
  • CPR required within two weeks of hire
  • MAP required within 150 days of hire
  • Safety Care required within 90 days of hire
  • Knowledge and use of advocacy, communication, empowerment, self-advocacy, physical personal management, household management, and crisis intervention techniques
  • Sensitivity to cultural, religious, racial, disability, and gender issues
  • Teaching skills
  • Ability to document interventions
  • Ability to work independently and as part of an interdisciplinary team
  • Ability to provide role model behaviors
  • Ability to perform physically active work, including standing, walking, bending, kneeling, stooping, crouching, crawling, and climbing
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to assist clients with tasks of daily living
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • Required close, distance, color, peripheral, and depth vision, plus ability to adjust focus
