About the role
Key responsibilities & impact- Support persons served in Vinfen settings who have been diagnosed with a brain injury
- Teach skills that enhance independence and quality of life
- Assist in developing, implementing, and documenting individual treatment support plans
- Perform rehabilitative and behavioral interventions and monitoring
- Schedule appointments and monitor progress
- Provide transportation and other related support
- Serve as a role model and help persons served become integrated into their communities
- Assess physical and emotional status and report changes to a supervisor
- Ensure program site safety and maintenance
- Participate in daily living and cleaning tasks, including cooking, meal planning, grocery shopping, housekeeping, lawn mowing, snow shoveling, and garbage removal
- Comply with corporate and divisional policies, safe work practices, workers’ compensation, universal precautions, and exposure control requirements
- Store, administer, document, and dispose of medication consistent with MAP requirements, funding-source regulations, and company policy
- Complete communication logs, community membership logs, chore schedules, timesheets, and application forms
- Perform crisis intervention as needed
- Provide current contact information to the Relief Pool Coordinator or Human Resources staff
- Perform other related duties as required
Requirements
What you’ll need- One to three months experience in a human services setting
- Experience working with individuals with brain injury preferred
- A high school diploma or equivalent is required
- Valid driver’s license must be presented at the time of employment if driving for Vinfen
- Drivers must be at least 21 years of age
- Drivers must have maintained a valid US driver’s license for at least six months
- Ability to pass a driver’s screening background check, subject to annual checks
- Safe driving record if driving
- Adequate insurance if using a personal vehicle
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways
- Ability to assist clients with tasks of daily living
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Close, distance, color, peripheral, and depth vision, and ability to adjust focus
- First Aid certification required within two weeks of hire
- CPR certification required within two weeks of hire
- MAP certification required within 150 days of hire
- Safety Care certification required within 90 days of hire
- Knowledge and use of advocacy, communication, empowerment, self-advocacy, personal management, household management, and crisis intervention techniques
- Sensitivity to cultural, religious, racial, disability, and gender issues
- Teaching skills
- Ability to document interventions
- Ability to work independently and as part of an interdisciplinary team
- Ability to provide role model behaviors
