About the role

Key responsibilities & impact
  • Provide support to persons served in Vinfen settings
  • Teach skills that enhance independence and quality of life
  • Assist in developing, implementing, and documenting individual treatment support plans
  • Perform rehabilitative and behavioral interventions and monitor progress
  • Schedule appointments and provide transportation
  • Provide role-model support and help persons served integrate into their communities
  • Assess physical and emotional status and report changes to a supervisor
  • Ensure program-site safety and maintenance
  • Participate in daily living and household tasks, including cooking, meal planning, grocery shopping, housekeeping, lawn mowing, snow shoveling, and garbage removal
  • Store, administer, document, and dispose of medication according to MAP requirements, regulations, and company policy
  • Complete communication logs, community membership logs, chore schedules, timesheets, and application forms
  • Perform crisis intervention as needed
  • Provide current contact information to the Relief Pool Coordinator or Human Resources
  • Perform other related duties as required

Requirements

What you’ll need
  • One to three months experience in a human services setting
  • In some cases, experience may be substituted for academic training
  • A high school diploma or equivalent is required
  • Valid driver’s license must be presented at the time of employment if driving
  • Must be at least 21 years of age if driving
  • Must have maintained a valid US driver’s license for at least six months if driving
  • Must pass a driver’s screening background check if driving
  • Must maintain a safe driving record if driving
  • Must possess and maintain adequate insurance if using a personal vehicle
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways
  • Ability to assist clients with tasks of daily living
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • Close, distance, color, peripheral, and depth vision, plus ability to adjust focus
  • First Aid required within two weeks of hire
  • CPR required within two weeks of hire
  • MAP required within 150 days of hire
