About the role
Key responsibilities & impact- Provide support to persons served in Vinfen settings
- Teach skills to enhance independence and quality of life
- Assist in developing, implementing, and documenting individual treatment support plans
- Perform daily rehabilitative and behavioral interventions and monitoring
- Schedule appointments and monitor progress
- Provide transportation and related support
- Serve as a role model and help persons served integrate into their communities
- Assess physical and emotional status and report changes to a supervisor
- Ensure program site safety and maintenance
- Oversee and participate in daily living and cleaning activities, including cooking, meal planning, grocery shopping, housekeeping, lawn mowing, snow shoveling, and garbage removal
- Comply with corporate and divisional policies and safe work practices
- Store, administer, document, and dispose of medication consistent with MAP requirements, regulations, and company policy
- Complete communication logs, community membership logs, chore schedules, timesheets, and application forms
- Perform crisis intervention as needed
- Provide current contact information to the Relief Pool Coordinator or Human Resources staff
- Perform other related duties as required
Requirements
What you’ll need- One to three months experience in a human services setting
- In some cases, experience may be substituted for academic training
- A high school diploma or equivalent is required
- A valid driver’s license must be presented at the time of employment
- Must be at least 21 years of age if driving
- Must have maintained a valid US driver’s license for at least six months if driving
- Must pass a driver’s screening background check if driving
- Must maintain a safe driving record if driving
- Must possess and maintain adequate insurance when using a personal vehicle
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways
- Ability to assist clients with tasks of daily living
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Required close, distance, color, peripheral, and depth vision, plus ability to adjust focus
- First Aid required within two weeks of hire
- CPR required within two weeks of hire
- MAP required within 150 days of hire
- Knowledge and use of advocacy techniques, communication styles, empowerment and self-advocacy techniques, physical personal management skills, household management skills, and crisis intervention techniques
- Sensitivity to cultural, religious, racial, disability, and gender issues
- Teaching skills
- Ability to document interventions
- Ability to work independently and as part of an interdisciplinary team
- Ability to provide role model behaviors
