About the role

Key responsibilities & impact
  • Promote engagement, housing permanency, community tenure, self-sufficiency, and successful completion of residence tenure for Persons Served
  • Manage GLE or SIE residential site systems, procedures, regulations, quality improvement, intakes, and discharges
  • Facilitate timely transitions to and from residential settings with the Housing Coordinator
  • Create a welcoming, respectful, and supportive residential environment for families, friends, and significant others
  • Coordinate Team and GLE/SIE staff to deliver Treatment Plan interventions
  • Ensure non-clinical Treatment Plan support interventions are completed under LPHA Clinician direction
  • Implement Safety Plans and Crisis Prevention and Response Plans
  • Monitor Treatment Plan documentation quality, timeliness, and Medicaid Rehab Option standards adherence
  • Monitor staff use of evidence-based practices and request additional training as needed
  • Manage DMH licensing adherence and Medication Administration Program compliance
  • Manage site resources, benefits, representative payee accounts, Client Funds, expenses, budgets, and billing information
  • Manage adverse events and Incident Reporting, ensuring corrective actions
  • Oversee Human Rights policies and procedures
  • Ensure residence maintenance, repair, safety, and good-neighbor relationships
  • Maintain alliances with other service providers and Care Coordination entities
  • Hire, orient, supervise, evaluate, and provide performance feedback to residential staff
  • Manage relief and overtime resources
  • Implement staff retention, recognition, awards, and career development initiatives
  • Ensure required residential Performance Measures are maintained
  • Support trauma-informed, recovery-oriented, person-centered, culturally competent, and strengths-based practices
  • Document Persons Served needs and services
  • Promote access to healthcare, employment, and education services
  • Participate in the regional GLE/SIE Program Director Community of Practice Workgroup
  • Provide on-call coverage as assigned
  • Perform other related duties as assigned

Requirements

What you’ll need
  • High school diploma or equivalent required; in some cases, experience may substitute for academic training
  • Minimum of one year of progressively responsible human services experience required
  • Experience working with people with psychiatric disability and/or co-occurring disorders required
  • Certified Psychiatric Rehabilitation Practitioner credential preferred
  • Management experience preferred
  • Access to an operational and insured vehicle and willingness to use it to transport Persons Served
  • Valid US driver's license required; maintained for at least six months
  • At least 21 years of age
  • Adequate personal-vehicle insurance if using a personal vehicle
  • Safe driving record and ability to pass a driver's screening background check
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, and assist Persons Served with activities of daily living
  • Ability to operate a computer and office equipment
  • Ability to speak, hear, and communicate with Persons Served, staff, and external representatives
  • CPR required within first two weeks of hire
  • First Aid required within two weeks of hire
  • MAP required within 150 days of hire
  • Knowledge of program management, operational management, administrative systems, housing assessment, documentation standards, data-driven decision-making, and personal computer applications
  • Ability to use or adhere to Motivational Interviewing, Harm Reduction, SBIRT, Housing First, Stages of Change, WRAP, Seeking Safety, and Cognitive Behavioral interventions
  • Ability to collaborate with team members and implement and train staff on non-clinical Treatment Plan interventions
