About the role
Key responsibilities & impact- Provide direct support to people with behavioral conditions and intellectual and developmental disabilities in residential settings
- Assist in developing, implementing, and documenting treatment plans, ISP/IPs, and clinical programs
- Provide rehabilitative and behavioral interventions and monitor residents
- Support appointment scheduling, money management, activities of daily living, grocery shopping, meal planning, cooking, and household chores
- Facilitate community integration through recreational and social activities, outings, community travel, church activities, shopping, and celebrations
- Provide transportation as necessary
- Assess residents’ physical and emotional status and report changes to a supervisor
- Monitor residents’ funds
- Maintain records, collect behavioral and academic data, write progress notes, and enter information into electronic health records
- Store, administer, document, and dispose of medications according to MAC requirements and applicable regulations
- Communicate with nurses, funding agencies, case managers, service providers, physicians, and other interested parties
- Participate in mandated training and staff meetings and assist with new-staff orientation
- Ensure compliance with internal and external regulatory requirements
- Maintain program-site safety and perform housekeeping, lawn mowing, snow shoveling, and garbage removal
- Complete paperwork, logs, timesheets, and online data records
- Maintain required staff-to-consumer ratios and provide last-minute coverage
- Serve as a human rights advocate and provide physical intervention or first aid during emergencies
- Review and respond to work emails and follow administrative policies
- Perform other related duties as required
Requirements
What you’ll need- At least one year of experience and/or training in a related function is preferred
- High school diploma or equivalent is required
- Valid US driver's license required
- At least 21 years of age
- Maintained a valid US driver's license for at least six months
- Adequate insurance and a safe driving record if using a personal vehicle
- Must pass a driver's screening background check
- CPR required within two weeks of hire
- First Aid required within two weeks of hire
- MAC follows CT guidelines
- Safety Care required within two weeks of hire
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, and climb stairs
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, printer, Microsoft Outlook, and an electronic health record
- Ability to assist clients with activities of daily living
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Required close, distance, color, peripheral, and depth vision and ability to adjust focus
- Knowledge of advocacy, communication, empowerment, self-advocacy, participatory planning, physical personal management, household management, and crisis intervention techniques
- Sensitivity to cultural, religious, racial, disability, and gender issues
- Participatory, collaborative, and teaching skills
- Ability to work independently and as part of an interdisciplinary team
