About the role

Key responsibilities & impact
  • Provide direct support to people with intellectual and developmental disabilities in residential settings
  • Assist with developing, implementing, and documenting treatment plans, ISP/IPs, and clinical programs
  • Provide rehabilitative and behavioral interventions and monitor residents
  • Support activities of daily living, household chores, grocery shopping, meal planning, and cooking
  • Schedule appointments and assist with money management
  • Plan recreational and social activities, outings, community travel, and other activities
  • Help individuals integrate into their communities and access community resources
  • Provide transportation as necessary
  • Assess residents’ physical and emotional status and report changes to supervisors
  • Monitor residents’ funds as needed
  • Maintain records, progress notes, communication logs, and electronic health records
  • Collect behavior and academic data
  • Store, administer, document, and dispose of medication according to MAC requirements and policies
  • Communicate with nurses, funding agencies, case managers, service providers, physicians, and other interested parties
  • Participate in mandated training and staff meetings and assist with new staff orientation
  • Ensure program site safety, maintenance, housekeeping, lawn mowing, snow shoveling, and garbage removal
  • Complete paperwork, logs, timesheets, and online data records
  • Maintain required staff-to-consumer ratios and provide last-minute coverage
  • Serve as a human rights advocate and provide physical intervention or first aid during emergencies
  • Review and respond to work emails and follow administrative policies

Requirements

What you’ll need
  • A high school diploma or equivalent is required
  • At least one year of experience and/or training in a related function is preferred
  • Valid driver's license required at employment
  • At least 21 years of age
  • Valid US driver's license maintained for at least six months
  • Adequate insurance and safe driving record if using a personal vehicle
  • Must pass a driver's screening background check
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer and office equipment
  • Ability to assist clients with activities of daily living
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • CPR certification required within two weeks of hire
  • First Aid certification required within two weeks of hire
  • MAC certification following Connecticut guidelines
  • Safety Care certification required within two weeks of hire
  • Knowledge of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, household management, and personal management techniques
  • Ability to work independently and as part of an interdisciplinary team
  • Ability to learn and use Microsoft Outlook and an electronic health record
