  • Maintain a safe, secure, clean, and well-maintained residential environment.
  • Support community integration, friendships, family engagement, and access to community resources.
  • Plan recreational and social activities promoting inclusion and emotional well-being.

  • Support work, volunteer, and other community engagement opportunities.
  • Provide training and support involving rehabilitation, behavioral interventions, fire and safety, appointments, money management, ADLs, shopping, meal planning, cooking, and household chores.
  • Perform hands-on tasks and provide transportation as necessary.
  • Assess physical and emotional status and report changes to the supervisor.
  • Monitor individual funds and maintain records, progress notes, data, communication logs, and activity schedules.
  • Procure, store, administer, document, and dispose of medication in accordance with MAP requirements, regulations, and company policy.
  • Communicate with nurses, case managers, visiting nurses, families, and other interested parties.
  • Attend mandated training and staff meetings.
  • Ensure compliance with regulatory policies, procedures, and standards.
  • Perform housekeeping, snow shoveling, trash removal, and home safety maintenance.
  • Provide shift coverage in other homes or programs as assigned.
  • Function as a human rights advocate and provide physical intervention or First Aid during emergencies.
  • Assist with developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs.
  • Perform other related duties as required.

Requirements

What you’ll need
  • Candidates with one or more years of experience and/or training in a related function are encouraged to apply.
  • A high school diploma or equivalent is required.
  • Valid driver's license required at employment.
  • At least 21 years of age.
  • Maintained a valid US driver's license for at least six months.
  • Pass a driver's screening background check.
  • Adequate insurance and a safe driving record required when using a personal vehicle.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces or hallways.
  • Must be able to lift at least 25 pounds using proper lifting techniques or a two-person lift.
  • Ability to operate a computer, calculator, copier, and printer.
  • Ability to assist clients with tasks of daily living.
  • Ability to speak, hear, and communicate with clients, staff, and external representatives.
  • Specific vision abilities including close, distance, color, peripheral, and depth vision, plus ability to adjust focus.
  • CPR required within two weeks of hire.
  • First Aid required within two weeks of hire.
  • MAP required within 150 days of hire.
  • PBS Tier 1 required within 150 days of hire.
  • Safety Care required within 90 days of hire.
  • May be asked to obtain CBIS certification (Certified Brain Injury Specialist).
