  • Provide individual and group support to people served in a residential program for individuals with brain injuries.
  • Teach skills that enhance independence, self-advocacy, and quality of life.
  • Maintain a safe, secure, clean, and well-maintained residential environment.
  • Help individuals integrate into their communities, develop and maintain friendships, and strengthen family partnerships.
  • Assist with access to community-based supports and resources.
  • Plan recreational and social activities promoting inclusion and emotional well-being.
  • Support access to assistive technology and adaptive equipment.
  • Support work, volunteer, and other community engagement opportunities.
  • Provide training and support involving rehabilitative and behavioral interventions, fire and safety regulations, appointments, money management, ADLs, shopping, meal planning, cooking, and household chores.
  • Perform hands-on tasks to implement support plans.
  • Provide transportation as necessary.
  • Assess physical and emotional status and report changes to the supervisor.
  • Monitor individual funds and maintain records, behavior and academic data, progress notes, communication logs, incident documentation, and activity schedules.
  • Procure, store, administer, document, and dispose of medication consistent with MAP requirements and applicable regulations and policies.
  • Communicate with program nurses, case managers, Visiting Nurses Association, families, and other interested parties.
  • Participate in mandated training and staff meetings.
  • Ensure compliance with regulatory policies, procedures, and standards.
  • Provide shift coverage in other homes or programs as assigned.
  • Function as a human rights advocate and provide physical intervention and/or First Aid during emergencies.
  • Serve as a reporter to the Disabled Persons Protections Commission.
  • Assist in developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs.
  • Perform general housekeeping, snow shoveling, trash removal, and other related duties.

Requirements

What you’ll need
  • Candidates with one or more years of experience and/or training in a related function will be given preference for employment.
  • A high school diploma or equivalent is required.
  • Valid US driver's license required at employment; must be at least 21 years old, have maintained a valid US driver's license for at least one year, and pass a driver's screening background check.
  • If using a personal vehicle, must possess and maintain adequate insurance and a safe driving record subject to annual checks.
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds using proper lifting techniques or a two-person lift, and assist clients with daily living tasks.
  • Ability to operate a computer and office equipment including a calculator, copier, and printer.
  • Ability to communicate with clients, staff, and external representatives.
  • CPR required within two weeks of hire.
  • First Aid required within two weeks of hire.
  • MAP required within 150 days of hire.
  • PBS Tier 1 required within 150 days of hire.
  • Safety Care required within 90 days of hire.
  • May be asked to obtain CBIS certification.
  • Knowledge and use of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, communication, household management, and physical personal management techniques.
  • Ability to work independently and as part of an interdisciplinary team.
  • Ability to communicate effectively in oral and written formats.
  • Sensitivity to cultural, religious, racial, disability, and gender issues.
