About the role
Key responsibilities & impact- Provide direct support to people with brain injury or behavioral conditions in residential settings
- Build independence skills and improve quality of life
- Administer medication under Medication Administration Plan (MAP) requirements
- Implement treatment plans, ISP/PSTPs, clinical programs, rehabilitative interventions, and behavioral interventions
- Assist with activities of daily living, household chores, cooking, meal planning, grocery shopping, and money management
- Support community integration, friendships, social inclusion, employment, volunteering, and community engagement
- Assist individuals with self-advocacy, decision-making, empowerment, assistive technology, and adaptive equipment
- Schedule appointments and provide transportation for outings and appointments
- Assess physical and emotional status and report changes to supervisors
- Monitor individual funds and maintain records, progress notes, behavior and academic data, incident documentation, communication logs, membership logs, and activity schedules
- Communicate with program nurses, case managers, Visiting Nurses Association, families, and other interested parties
- Participate in mandated training and staff meetings
- Ensure compliance with regulatory policies, procedures, standards, and safe work practices
- Maintain home safety and cleanliness, including housekeeping, snow shoveling, and trash removal
- Provide shift coverage in other homes or programs as assigned
- Function as a human rights advocate and provide physical intervention or First Aid during emergencies
- Serve as a reporter to the Disabled Persons Protections Commission
- Perform other related duties as required
Requirements
What you’ll need- High school diploma or GED required
- Bachelor's degree in a Human Services related field preferred
- Experience working with people with developmental disabilities, behavioral conditions and/or brain injury strongly preferred
- Strong time management and writing skills
- Must be at least 21 years of age
- Current, valid US driver's license; license held for at least 6 months in one section and at least one year in the driving requirements section
- Ability to obtain CPR and First Aid certification within 2 weeks of employment
- Ability to obtain MAP certification within 150 days of employment
- Ability to obtain PBS Tier 1 certification within 150 days of employment
- Ability to obtain Safety Care certification within 90 days of employment
- Must successfully pass CORI, reference, national fingerprinting, driving record, and driver's screening checks
- Ability to maintain adequate insurance and a safe driving record when using a personal vehicle
- One or more years of related experience and/or training preferred for employment
- Knowledge and use of advocacy, empowerment, self-advocacy, communication, participatory planning, crisis intervention, physical personal management, and household management techniques
- Ability to promote family partnerships, social inclusion, individual choice, and implement PBS treatment programs
- Ability to work independently and as part of an interdisciplinary team
- Ability to communicate effectively in oral and written formats
- Ability to work professionally and confidentially
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, operate office equipment, assist clients with daily living tasks, and communicate with clients and staff
