Key responsibilities & impact

  • Administer medication under the Medication Administration Plan (MAP)
  • Implement treatment plans and assist with activities of daily living
  • Assist individuals with household chores, cooking, meal planning, grocery shopping, and money management
  • Support community integration, friendships, social inclusion, and access to community resources
  • Plan recreational and social activities
  • Promote self-advocacy, choice-making, empowerment, and participation in support decisions
  • Support access to assistive technology and adaptive equipment
  • Support work, volunteer, and community engagement opportunities
  • Provide rehabilitative and behavioral interventions and monitoring
  • Provide transportation for outings, shopping, and medical appointments
  • Assess physical and emotional status and report changes to the supervisor
  • Maintain individual records, behavior and academic data, progress notes, communication logs, and activity schedules
  • Procure, store, administer, document, and dispose of medication according to MAP requirements and policy
  • Communicate with nurses, case managers, Visiting Nurses Association, families, and other interested parties
  • Participate in mandated training and staff meetings
  • Ensure regulatory, safety, housekeeping, maintenance, and corporate policy compliance
  • Provide shift coverage in other homes or programs as assigned
  • Serve as a human rights advocate and provide physical intervention or First Aid during emergencies
  • Assist with developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs

Requirements

What you’ll need
  • One or more years of experience and/or training in a related function preferred
  • High school diploma or equivalent required
  • Valid driver's license required at employment
  • At least 21 years of age
  • Valid US driver's license maintained for at least one year
  • Ability to pass a driver's screening background check
  • Adequate insurance and safe driving record required when using a personal vehicle
  • CPR required within two weeks of hire
  • First Aid required within two weeks of hire
  • MAP required within 150 days of hire
  • PBS Tier 1 required within 150 days of hire
  • Safety Care required within 90 days of hire
  • May be asked to obtain CBIS certification
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces or hallways
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to assist clients with activities of daily living
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • Knowledge of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, household management, and physical personal management techniques
  • Ability to work independently and as part of an interdisciplinary team
  • Ability to communicate effectively in oral and written formats
  • Sensitivity to cultural, religious, racial, disability, and gender issues
