About the role
Key responsibilities & impact- Provide individual and group support to people served in a residential program
- Teach skills that enhance independence, self-advocacy, and quality of life
- Maintain a safe, secure, and clean residential environment
- Support community integration, social inclusion, friendships, family partnerships, and community engagement
- Help individuals access community-based support, resources, assistive technology, and adaptive equipment
- Plan recreational and social activities
- Support employment, volunteer, and other community opportunities
- Provide training and support involving behavioral and rehabilitative interventions, fire and safety regulations, appointments, money management, activities of daily living, shopping, meal planning, cooking, and household chores
- Perform hands-on tasks to implement support plans
- Provide transportation as necessary
- Assess physical and emotional status and report changes to the supervisor
- Monitor individual funds
- Maintain records, progress notes, communication logs, behavior data, academic data, incident documentation, and activity schedules
- Procure, store, administer, document, and dispose of medication in accordance with MAP requirements, regulations, and company policy
- Communicate with program nurses, case managers, Visiting Nurses Association, and other interested parties
- Participate in mandated training and staff meetings
- Ensure compliance with internal and external regulatory policies and standards
- Maintain home safety and perform housekeeping, snow shoveling, and trash removal
- Provide shift coverage in other homes or programs as assigned
- Function as a human rights advocate and provide physical intervention or First Aid during emergencies
- Report to the Disabled Persons Protections Commission when required
- Assist with developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs
Requirements
What you’ll need- One or more years of experience and/or training in a related function encouraged
- High school diploma or equivalent required
- Valid driver's license required at employment
- At least 21 years of age
- Valid US driver's license maintained for at least six months
- Adequate insurance and safe driving record if using a personal vehicle
- Must pass a driver's screening background check
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to assist clients with activities of daily living
- Ability to operate a computer, calculator, copier, and printer
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- CPR required within two weeks of hire
- First Aid required within two weeks of hire
- MAP required within 150 days of hire
- PBS Tier 1 required within 150 days of hire
- Safety Care required within 90 days of hire
- Knowledge and use of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, household management, and physical personal management techniques
- Ability to work independently and as part of an interdisciplinary team
- Ability to communicate effectively in oral and written formats
- Sensitivity to cultural, religious, racial, disability, and gender issues
