About the role
Key responsibilities & impact- Provide individual and group support to people served in a residential program
- Teach skills that enhance independence, self-advocacy, and quality of life
- Ensure a safe, secure, and clean residential environment
- Support community integration, friendships, family engagement, and social inclusion
- Assist individuals in accessing community-based support and resources
- Plan recreational and social activities
- Support access to assistive technology and adaptive equipment
- Support work, volunteer, and community engagement opportunities
- Provide training in rehabilitative and behavioral interventions, fire and safety, appointments, money management, ADLs, shopping, meal planning, cooking, and household chores
- Perform hands-on tasks and provide transportation as necessary
- Assess physical and emotional status and report changes to the supervisor
- Monitor individual funds and maintain records, progress notes, behavior and academic data, communication logs, and activity schedules
- Procure, store, administer, document, and dispose of medication according to MAP requirements, regulations, and company policy
- Communicate with program nurses, case managers, and Visiting Nurses Association
- Participate in mandated training and staff meetings
- Ensure compliance with regulatory policies, procedures, and standards
- Maintain home safety and perform housekeeping, snow shoveling, and trash removal
- Provide shift coverage in other homes/programs as assigned
- Function as a human rights advocate and provide physical intervention or First Aid during emergencies
- Serve as a reporter to the Disabled Persons Protections Commission
- Assist in developing, implementing, and documenting ISP/PSTPs and clinical programs
- Perform other related duties as required
Requirements
What you’ll need- One or more years of experience and/or training in a related function encouraged
- High school diploma or equivalent required
- Experience may be substituted for academic training in some cases
- Valid driver's license required at employment
- Must be at least 21 years of age
- Must have maintained a valid US driver's license for at least six months
- Adequate insurance and safe driving record required when using a personal vehicle
- Must pass a driver's screening background check
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
- Must be able to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to sit, reach, climb stairs, and maneuver through narrow spaces or hallways
- Ability to assist clients with activities of daily living
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Required vision abilities include close, distance, color, peripheral, and depth perception and ability to adjust focus
- CPR required within two weeks of hire
- First Aid required within two weeks of hire
- MAP required within 150 days of hire
- PBS Tier 1 required within 150 days of hire
- Safety Care required within 90 days of hire
- Knowledge of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, household management, and physical personal management skills
- Ability to work independently and as part of an interdisciplinary team
- Ability to communicate effectively in oral and written formats
