  • Provide training and support in behavioral and rehabilitative interventions, safety, appointments, money management, ADLs, shopping, meal planning, cooking, and household chores
  • Perform hands-on tasks and provide transportation as necessary
  • Assess physical and emotional status and report changes to the supervisor
  • Monitor individual funds and maintain records, progress notes, behavior and academic data, incident documentation, and communication logs
  • Procure, store, administer, document, and dispose of medication according to MAP requirements and applicable policies
  • Communicate with nurses, case managers, the Visiting Nurses Association, and other interested parties
  • Participate in mandated training and staff meetings
  • Ensure compliance with regulatory policies, procedures, and standards
  • Perform housekeeping, snow shoveling, trash removal, and home maintenance
  • Complete paperwork and logs and provide shift coverage at other homes/programs as assigned
  • Function as a human rights advocate and provide physical intervention or First Aid during emergencies
  • Assist with developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs
  • Perform other related duties as required

Requirements

What you’ll need
  • One or more years of experience and/or training in a related function preferred for employment
  • High school diploma or equivalent required
  • Valid driver's license required at employment
  • At least 21 years of age
  • Valid US driver's license maintained for at least six months
  • Ability to pass a driver's screening background check
  • Adequate insurance and safe driving record if using a personal vehicle
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, and assist individuals with activities of daily living
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to communicate effectively in oral and written formats
  • Knowledge of advocacy, communication, empowerment, self-advocacy, participatory planning, household management, and crisis intervention techniques
  • Ability to work independently and as part of an interdisciplinary team
  • Sensitivity to cultural, religious, racial, disability, and gender issues
  • CPR certification required within two weeks of hire
  • First Aid certification required within two weeks of hire
  • MAP certification required within 150 days of hire
  • PBS Tier 1 certification required within 150 days of hire
  • Safety Care certification required within 90 days of hire
