About the role
Key responsibilities & impact- Provide individual and group support to people served in a residential program.
- Teach skills that enhance independence, self-advocacy, and quality of life.
- Maintain a safe, secure, clean residential environment.
- Help individuals integrate into their communities and develop and maintain friendships.
- Help individuals access community-based supports and resources.
- Plan recreational and social activities promoting social inclusion and emotional well-being.
- Support individuals in making choices and directing their supports and lives.
- Support access to assistive technology and adaptive equipment.
- Support work, volunteer, and community engagement opportunities.
- Provide training and support including behavioral interventions, fire and safety regulations, appointment scheduling, money management, ADLs, grocery shopping, meal planning, cooking, and household chores.
- Perform hands-on tasks and provide transportation as necessary.
- Assess physical and emotional status and report changes to the supervisor.
- Monitor individual funds.
- Maintain records, collect behavior and academic data, write progress notes, and document incidents and activities.
- Procure, store, administer, document, and dispose of medication consistent with MAP requirements and applicable regulations.
- Communicate with program nurses, case managers, and Visiting Nurses Association representatives.
- Participate in mandated training and staff meetings.
- Ensure compliance with regulatory policies, procedures, and standards.
- Perform housekeeping, snow shoveling, trash removal, and home maintenance.
- Complete communication logs, community membership logs, and activity schedules.
- Provide shift coverage in other homes or programs as assigned.
- Function as a human rights advocate and provide physical intervention or First Aid during emergencies.
- Assist with developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs.
- Perform other related duties as required.
Requirements
What you’ll need- Candidates with one or more years of experience and/or training in a related function will be given preference for employment.
- A high school diploma or equivalent is required.
- Experience may be substituted for academic training in some cases.
- Driving is required using either a Vinfen van or personal vehicle.
- Personal vehicle users must possess and maintain adequate insurance and a safe driving record subject to annual checks.
- A valid US driver's license must be presented at employment.
- Must be at least 21 years of age.
- Must have maintained a valid US driver's license for at least six months.
- Must pass a driver's screening background check.
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, and assist people served with daily living tasks.
- Ability to operate a computer and office equipment including a calculator, copier, and printer.
- Ability to communicate effectively orally and in writing.
- CPR required within two weeks of hire.
- First Aid required within two weeks of hire.
- MAP required within 150 days of hire.
- PBS Tier 1 required within 150 days of hire.
- Safety Care required within 90 days of hire.
- Knowledge of advocacy, communication, empowerment, self-advocacy, participatory planning, personal management, household management, and crisis intervention techniques.
- Ability to work independently and as part of an interdisciplinary team.
- Ability to work professionally and confidentially.
- Sensitivity to cultural, religious, racial, disability, and gender issues.
