About the role
Key responsibilities & impact- Provide individual and group support to people served in a residential program
- Teach skills supporting independence, self-advocacy, quality of life, and community integration
- Maintain a safe, secure, clean, and well-maintained residential environment
- Help individuals access community-based supports, resources, friendships, work, volunteer, and community engagement opportunities
- Plan recreational and social activities promoting inclusion and emotional well-being
- Support choice-making, decision-making, family partnerships, and participation in support planning
- Assist with assistive technology and adaptive equipment
- Provide training and support involving behavioral and rehabilitative interventions, safety regulations, appointments, money management, ADLs, shopping, meal planning, cooking, chores, and generic resources
- Perform hands-on tasks and provide transportation as necessary
- Assess physical and emotional status and report changes to the supervisor
- Monitor individual funds and maintain records, progress notes, behavior and academic data, incident documentation, communication logs, schedules, and other paperwork
- Procure, store, administer, document, and dispose of medication according to MAP requirements, regulations, and company policy
- Communicate with program nurses, case managers, Visiting Nurses Association, families, and other interested parties
- Participate in mandated training and staff meetings
- Ensure compliance with regulatory, corporate, divisional, safety, workers’ compensation, universal precautions, and exposure-control policies
- Complete housekeeping, snow shoveling, trash removal, and other home safety and maintenance tasks
- Provide shift coverage in other homes or programs as assigned
- Function as a human rights advocate and provide physical intervention or First Aid during behavioral or medical emergencies
- Report to the Disabled Persons Protections Commission as required
- Assist in developing, implementing, and documenting ISPs/PSTPs and clinical programs
- Perform other related duties as required
Requirements
What you’ll need- One or more years of experience and/or training in a related function encouraged
- High school diploma or equivalent required; in some cases, experience may be substituted for academic training
- Valid driver's license required at employment
- At least 21 years of age
- Valid US driver's license maintained for at least six months
- Adequate insurance and safe driving record if using a personal vehicle
- Must pass a driver's screening background check
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to assist clients with activities of daily living
- Ability to operate a computer, calculator, copier, and printer
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Required vision abilities including close, distance, color, peripheral, and depth perception, plus ability to adjust focus
- CPR required within two weeks of hire
- First Aid required within two weeks of hire
- MAP required within 150 days of hire
- PBS Tier 1 required within 150 days of hire
- Safety Care required within 90 days of hire
- Knowledge and use of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, communication, household management, and physical personal management techniques
- Ability to work independently and as part of an interdisciplinary team
- Ability to communicate effectively in oral and written formats
