About the role
Key responsibilities & impact- Provide individual and group support to people served in a residential program
- Teach skills that enhance independence, self-advocacy, and quality of life
- Maintain a safe, secure, clean, and well-maintained residential environment
- Assist individuals with community integration, friendships, family engagement, and community-based resources
- Plan recreational and social activities promoting inclusion and emotional well-being
- Support personal choice, decision-making, self-advocacy, employment, volunteering, and community engagement
- Provide training and support involving behavioral and rehabilitative interventions, safety regulations, appointments, money management, ADLs, shopping, meal planning, cooking, and household chores
- Support access to assistive technology and adaptive equipment
- Provide transportation as necessary
- Assess physical and emotional status and report changes to the supervisor
- Monitor individual funds and maintain records, progress notes, behavior and academic data, incident documentation, communication logs, activity schedules, and community membership logs
- Procure, store, administer, document, and dispose of medication in accordance with MAP requirements and applicable regulations
- Communicate with nurses, case managers, Visiting Nurses Association, families, and other interested parties
- Participate in mandated training and staff meetings
- Ensure compliance with internal and external regulatory policies and standards
- Provide housekeeping, snow shoveling, trash removal, and other home maintenance
- Provide shift coverage in other homes or programs as assigned
- Serve as a human rights advocate and provide physical intervention or First Aid during emergencies
- Assist in developing, implementing, and documenting ISP/PSTPs and clinical programs
- Perform other related duties as required
Requirements
What you’ll need- One or more years of experience and/or training in a related function preferred
- High school diploma or equivalent required
- Valid driver's license required at employment
- At least 21 years of age
- Valid US driver's license maintained for at least six months
- Adequate insurance and safe driving record required when using a personal vehicle
- Ability to pass a driver's screening background check
- CPR required within two weeks of hire
- First Aid required within two weeks of hire
- MAP required within 150 days of hire
- PBS Tier 1 required within 150 days of hire
- Safety Care required within 90 days of hire
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, lift at least 25 pounds, and assist with activities of daily living
- Ability to operate a computer, calculator, copier, and printer
- Ability to communicate effectively in oral and written formats
- Knowledge of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, household management, and physical personal management skills
- Ability to work independently and as part of an interdisciplinary team
- Ability to work professionally and confidentially
- Sensitivity to cultural, religious, racial, disability, and gender issues
