About the role
Key responsibilities & impact- Provide individual and group support to people served in a residential program
- Teach skills that enhance independence, self-advocacy, quality of life, and community participation
- Maintain a safe, secure, clean, and well-maintained residential environment
- Help individuals access community-based support and resources
- Plan recreational and social activities promoting inclusion and emotional well-being

- Provide training and support in behavioral interventions, fire and safety, appointments, money management, ADLs, shopping, meal planning, cooking, and household chores
- Perform hands-on tasks and implement support plans

- Assess physical and emotional status and report changes to the supervisor
- Monitor individual funds
- Maintain records, progress notes, behavior and academic data, incident documentation, communication logs, community membership logs, and activity schedules
- Procure, store, administer, document, and dispose of medication according to MAP requirements and applicable policies
- Communicate with nurses, case managers, Visiting Nurses Association, families, and other interested parties
- Participate in mandated training and staff meetings
- Ensure compliance with regulatory, corporate, and divisional policies and procedures
- Provide shift coverage in other homes or programs as assigned
- Function as a human rights advocate and provide physical intervention or First Aid during emergencies
- Report to the Disabled Persons Protections Commission when required
- Assist in developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs
- Perform housekeeping, snow shoveling, trash removal, and other related duties
Requirements
What you’ll need- One or more years of experience and/or training in a related function encouraged
- High school diploma or equivalent required; in some cases, experience may be substituted for academic training
- Valid driver's license required at employment
- Must be at least 21 years of age
- Must have maintained a valid US driver's license for at least six months
- Must pass a driver's screening background check
- Adequate insurance and safe driving record required when using a personal vehicle
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces or hallways
- Must be able to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to operate a computer, calculator, copier, and printer
- Ability to assist clients with activities of daily living
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Specific vision abilities: close, distance, color, peripheral, and depth perception, and ability to adjust focus
- CPR required within two weeks of hire
- First Aid required within two weeks of hire
- MAP required within 150 days of hire
- PBS Tier 1 required within 150 days of hire
- Safety Care required within 90 days of hire
- Knowledge and use of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, physical personal management, and household management techniques
- Ability to work independently and as part of an interdisciplinary team
- Ability to communicate effectively in oral and written formats
