  • Provide training and support involving rehabilitative and behavioral interventions, fire and safety, appointments, money management, activities of daily living, shopping, cooking, chores, and generic resources
  • Perform hands-on tasks to implement support plans
  • Provide transportation as necessary
  • Assess physical and emotional status and report changes to the supervisor
  • Monitor individual funds and maintain records, progress notes, behavior and academic data, communication logs, incident documentation, and activity schedules
  • Procure, store, administer, document, and dispose of medication consistent with MAP requirements and regulations
  • Communicate with program nurses, case managers, Visiting Nurses Association, families, and other interested parties
  • Participate in mandated training and staff meetings
  • Ensure compliance with regulatory policies, procedures, and standards
  • Maintain home safety and perform housekeeping, snow shoveling, and trash removal
  • Provide shift coverage in other homes or programs as assigned
  • Function as a human rights advocate and provide physical intervention or First Aid during behavioral or medical emergencies
  • Serve as a reporter to the Disabled Persons Protections Commission
  • Assist in developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs

Requirements

What you’ll need
  • One or more years of experience and/or training in a related function are encouraged
  • A high school diploma or equivalent is required; in some cases, experience may be substituted for academic training
  • Driving is required using either a Vinfen van or personal vehicle
  • Valid driver's license presented at time of employment
  • At least 21 years of age
  • Maintained a valid US driver's license for at least six months
  • Ability to pass a driver's screening background check
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to assist clients with tasks of daily living
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • CPR required within two weeks of hire
  • First Aid required within two weeks of hire
  • MAP required within 150 days of hire
  • PBS Tier 1 required within 150 days of hire
  • Safety Care required within 90 days of hire
