  • Plan recreational and social activities promoting social inclusion and emotional well-being
  • Support individual choice-making and participation in decisions about their supports and lives
  • Assist with assistive technology and adaptive equipment
  • Support work, volunteer, and community engagement opportunities
  • Encourage strong partnerships among individuals, families, staff, and other parties
  • Provide training and support involving behavioral and rehabilitative interventions, fire and safety, appointments, money management, activities of daily living, shopping, cooking, chores, and generic resources
  • Perform hands-on tasks to implement support plans
  • Provide transportation as necessary
  • Assess physical and emotional status and report changes to the supervisor
  • Monitor individual funds
  • Maintain records, progress notes, communication logs, behavior data, academic data, incident documentation, and activity schedules
  • Procure, store, administer, document, and dispose of medication in accordance with MAP requirements and applicable regulations
  • Communicate with program nurses, case managers, and Visiting Nurses Association representatives
  • Participate in mandated training and staff meetings
  • Ensure compliance with regulatory policies, procedures, and standards
  • Perform home safety and maintenance, housekeeping, snow shoveling, and trash removal
  • Provide shift coverage in other homes or programs as assigned
  • Function as a human rights advocate and provide physical intervention or First Aid during emergencies
  • Report to the Disabled Persons Protections Commission when required
  • Assist in developing, implementing, and documenting ISP/PSTPs and clinical programs
  • Perform other related duties as required

Requirements

What you’ll need
  • One or more years of experience and/or training in a related function encouraged
  • High school diploma or equivalent required
  • In some cases, experience may be substituted for academic training
  • Valid driver's license required at employment
  • At least 21 years of age
  • Valid US driver's license maintained for at least six months
  • Adequate insurance and safe driving record if using a personal vehicle
  • Ability to pass a driver's screening background check
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, and climb
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to assist clients with tasks of daily living
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • CPR required within two weeks of hire
  • First Aid required within two weeks of hire
  • MAP required within 150 days of hire
  • PBS Tier 1 required within 150 days of hire
  • Safety Care required within 90 days of hire
  • Knowledge of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, household management, and physical personal management techniques
  • Ability to communicate effectively in oral and written formats
  • Ability to work independently and as part of an interdisciplinary team
  • Sensitivity to cultural, religious, racial, disability, and gender issues
