  • Provide training and support in rehabilitative and behavioral interventions, fire and safety regulations, appointment scheduling, money management, ADLs, shopping, meal planning, cooking, household chores, and generic resources
  • Perform hands-on tasks to implement support plans
  • Provide transportation as necessary
  • Assess physical and emotional status and report changes to the supervisor
  • Monitor individual funds
  • Maintain records, collect behavior and academic data, write progress notes, and document incidents and activities
  • Procure, store, administer, document, and dispose of medication consistent with MAP requirements, regulations, and company policy
  • Communicate with program nurses, case managers, and Visiting Nurses Association
  • Participate in mandated training and staff meetings
  • Ensure compliance with regulatory policies, procedures, and standards
  • Maintain home safety and maintenance, including housekeeping, snow shoveling, and trash removal
  • Complete communication logs, community membership logs, activity schedules, and other paperwork
  • Provide shift coverage in other homes/programs as assigned
  • Function as human rights advocate and provide physical intervention or First Aid during behavioral or medical emergencies
  • Serve as a reporter to the Disabled Persons Protections Commission
  • Assist in developing, implementing, and documenting treatment, including ISP/PSTPs and clinical programs
  • Perform other related duties as required

Requirements

What you’ll need
  • One or more years of experience and/or training in a related function preferred for employment
  • High school diploma or equivalent required
  • Valid US driver's license presented at employment
  • At least 21 years of age
  • Maintained a valid US driver's license for at least six months
  • Pass a driver's screening background check
  • Maintain adequate insurance when using a personal vehicle
  • Maintain a safe driving record subject to annual checks
  • Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces or hallways
  • Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
  • Ability to assist clients with tasks of daily living
  • Ability to operate a computer, calculator, copier, and printer
  • Ability to speak, hear, and communicate with clients, staff, and external representatives
  • Ability to perform cleaning, meal preparation, vacuuming, shoveling, and grocery shopping
  • CPR required within two weeks of hire
  • First Aid required within two weeks of hire
  • MAP required within 150 days of hire
  • PBS Tier 1 required within 150 days of hire
  • Safety Care required within 90 days of hire
