About the role
Key responsibilities & impact- Provide individual and group support to people served in a residential program
- Teach skills that enhance independence, self-advocacy, and quality of life
- Maintain a safe, secure, and clean residential environment
- Help individuals integrate into their communities and develop and maintain friendships
- Assist with community-based support and resources
- Plan recreational and social activities promoting inclusion and emotional well-being
- Support individual choice-making and participation in decisions about supports and lives
- Provide access to assistive technology and adaptive equipment
- Support work, volunteer, and community engagement opportunities
- Promote strong partnerships with families, staff, and other interested parties
- Provide training and support including rehabilitative and behavioral interventions, fire and safety, appointments, money management, ADLs, shopping, meal planning, cooking, and household chores
- Perform hands-on tasks required to implement support plans
- Provide transportation as necessary
- Assess physical and emotional status and report changes to the supervisor
- Monitor individual funds
- Maintain records, progress notes, behavior and academic data, incident documentation, communication logs, community membership logs, and activity schedules
- Procure, store, administer, document, and dispose of medication consistent with MAP requirements, regulations, and company policy
- Communicate with program nurses, case managers, and Visiting Nurses Association
- Participate in mandated training and staff meetings
- Ensure compliance with internal and external policies, procedures, and standards
- Maintain home safety and perform housekeeping, snow shoveling, and trash removal
- Provide shift coverage in other homes/programs as assigned
- Function as a human rights advocate and provide physical intervention or First Aid during emergencies
- Serve as a reporter to the Disabled Persons Protections Commission
- Assist in developing, implementing, and documenting treatment, ISP/PSTPs, and clinical programs
Requirements
What you’ll need- One or more years of experience and/or training in a related function encouraged
- High school diploma or equivalent required; in some cases, experience may be substituted for academic training
- Valid driver's license presented at time of employment
- At least 21 years of age
- Valid US driver's license maintained for at least six months
- Adequate insurance and a safe driving record if using a personal vehicle
- Pass a driver's screening background check
- Ability to stand, walk, bend, kneel, stoop, crouch, crawl, climb, sit, reach, climb stairs, and maneuver through narrow spaces or hallways
- Ability to lift at least 25 pounds using proper lifting techniques or a two-person lift
- Ability to assist clients with activities of daily living
- Ability to speak, hear, and communicate with clients, staff, and external representatives
- Ability to operate a computer, calculator, copier, and printer
- CPR required within two weeks of hire
- First Aid required within two weeks of hire
- MAP required within 150 days of hire
- PBS Tier 1 required within 150 days of hire
- Safety Care required within 90 days of hire
- Knowledge of advocacy, empowerment, self-advocacy, participatory planning, crisis intervention, household management, and physical personal management techniques
- Ability to use different communication styles and communicate effectively orally and in writing
- Ability to work independently and as part of an interdisciplinary team
- Sensitivity to cultural, religious, racial, disability, and gender issues
